Main duties of the job

The main duties of the Practice Nurse include delivering high-quality, patient-centered care within the GP practice setting. Key responsibilities involve chronic disease management (such as asthma, diabetes, and cardiovascular disease), administering immunizations and vaccinations, wound care, cervical screening, health promotion, and providing family planning advice. The Practice Nurse will also manage minor illness clinics, conduct routine health checks, and support infection control measures, all while working collaboratively with the wider healthcare team to ensure the best possible outcomes for patients

**Practice Nurse Job Description**

**Job Title:** Practice Nurse

**Reports to:** Practice Manager/Clinical Lead

**Location:** Market Street Medical Practice

**Job Purpose:**

The Practice Nurse will work as part of the primary healthcare team providing a range of clinical services and ensuring high-quality patient care. The role involves autonomous clinical practice, chronic disease management, health promotion, minor illness treatment, and participation in care planning.

**Main Responsibilities:**

1. **Clinical Duties**

- Provide assessment, screening, treatment, and care to patients, including management of chronic diseases such as asthma, diabetes, and hypertension.

- Administer vaccinations and immunizations as per national and practice guidelines.

- Carry out wound management and dressings, minor illness triage, cervical smears, and sexual health and family planning advice.

- Conduct health screenings and routine checks, including NHS health checks, lifestyle management, and other assessments.

- Provide travel health advice and vaccinations.

2. **Health Promotion**

- Offer advice on diet, exercise, smoking cessation, and disease prevention.

- Support patients in developing self-management plans for long-term conditions.

3. **Care Management**

- Participate in the care planning process for patients, involving multi-disciplinary team discussions as needed.

- Conduct regular reviews and update patient records using the practices electronic system, ensuring accuracy and confidentiality at all times.

4. **Team Collaboration**

- Work collaboratively with GPs, healthcare assistants, and other practice staff to deliver the best care for patients.

- Contribute to clinical meetings, audits, and the development of protocols and guidelines for the practice.

5. **Clinical Governance**

- Ensure all care delivered meets CQC standards and adheres to current clinical guidelines and evidence-based practice.

- Engage in ongoing professional development and training to maintain competency and remain up-to-date with clinical advancements and practice needs.

6. **Patient Care and Communication**

- Build and maintain positive patient relationships, respecting diversity and patient rights.

- Deliver care in a compassionate, empathetic, and effective manner, focusing on patient-centered service.

7. **Quality Assurance**

- Participate in clinical audits, peer reviews, and continuous quality improvement initiatives to enhance patient care and practice performance.

**Essential Skills and Qualifications:**

- Registered Nurse with NMC registration.

- Proven experience in primary care, ideally in a GP practice setting.

- Competence in chronic disease management and evidence of continuous professional development in this area.

- Proficiency in administering vaccinations and providing screening services.

- Strong communication and interpersonal skills with a commitment to high standards of care.

**Desirable Attributes:**

- Additional qualifications in minor illness management, family planning, and/or smoking cessation.

- Knowledge of relevant policies, guidelines, and compliance requirements in primary care.

Essential Qualifications and Experience: Registered Nurse with a valid NMC (Nursing and Midwifery Council) registration. Experience in a primary care or community setting. Proven competence in chronic disease management, including asthma, diabetes, cardiovascular disease, and other long-term conditions. Experience in administering immunizations, vaccinations, and performing cervical smears. Evidence of continuous professional development relevant to the role.

Desirable

Ability to assess, plan, implement, and evaluate patient care autonomously. Proficiency in wound care, including dressings and minor injury management. Strong clinical skills in areas such as minor illness, family planning, and health screening. Excellent communication and interpersonal skills to establish effective relationships with patients, colleagues, and external stakeholders. IT literacy, with experience using clinical software systems and accurate record-keeping skills.
