Job summary

Main duties of the job

The post-holderwill deal with a wide range of health needs in a primarycare setting.

Clinicalresponsibilities:

As agreed, thepost-holder will make themselves available to undertake a variety of duties,including surgery consultations, telephone consultations and queries, checking/signing repeat prescriptions and dealing withqueries, paperwork and correspondence in a timely fashion

Makingprofessional, autonomous decisions in relation to presenting problems, whetherself-referred or referred from other health care workers within theorganisation

Screeningpatients for disease risk factors and early signs of illness

Developing careplans for health in consultation with patients and in line with currentpractice disease management protocols

Recording clearand contemporaneous consultation notes to agreed standards

Compiling andissuing computer-generated acute and repeat prescriptions

In general, thepost-holder will be expected to undertake all the normal duties andresponsibilities associated with a GP working within primary care.

Full primary care multidisciplinary team consisting of 14 GP Partners, 3 Salaried GPs ANP, 2 MSK Practitioners, 6 Paramedics, 3 Clinical Pharmacists,2 Pharmacy Technicians, 2 Mental Health Practitioners, 2 Social Prescribers and a Health and Well Being Coach.

Paramedic run home visiting service, very rare GP need for home visiting.

Full workflow optimisation through highly skilled admin team.

We make up 66% of a cohesive three practice PCN.

Easily commutable from Portsmouth/Southsea, two hours from London.

Job description

Job responsibilities

a.Thedelivery of highly effective medical care to the entitled population

b.Theprovision of services commensurate with the GMS contract

c.Genericprescribing adhering to local and national guidance

d.Effectivemanagement of long-term conditions

e.Processingof administration in a timely manner, including referrals, repeat prescriptionrequests and other associated administrative tasks

f.Ona rotational basis, undertake telephone triage and duty doctor roles

g.Maintainaccurate clinical records in conjunction with good practice, policy andguidance

h.Workcollaboratively, accepting an equal share of the practice workload

i.Adhereto best practice recommended through clinical guidelines and the audit process

j.Contributeto the successful implementation of continuous improvement and qualityinitiatives within the practice

k.Acceptdelegated responsibility for a specific area (or areas) or the QOF

l.Attendand contribute effectively to practice meetings as required

m.Contributeeffective to the development and maintenance of the practice including clinicalgovernance and training

n.Ensurecompliance with the appraisal process

o.Prepareand complete the revalidation process

p.Committo self-learning and instil an ethos of continuing professional developmentacross the practice team

q.Supportthe training of medical students from all clinical disciplines

r.Supportthe partners in achieving the strategic aims of the practice, makingrecommendations to enhance income and reduce expenditure

Qualifications

Essential

Qualified GP MRCGP Vocational Training Certificate or equivalent JCPTGP General Practitioner (Certificate of Completion of Training CCT) Full GMC registration National Performers List registration Appropriate defence indemnity (MPS/MDU) Eligibility to practice in the UK independently
