Job summary
We are looking for an innovative and highly motivated Community Nursery Nurse/Support Assistant to join our SEND support team across Kingston and Richmond for the 0-19 years age range.
This is a unique and exciting position working alongside the specialist practitioners supporting families in the period leading up to assessment and post diagnosis of any complex health need, including those diagnosed with Autistic Spectrum Condition. You will be offering practical and emotional support with areas of concern which are having a significant impact on the family. This is a varied role including, but not limited to
Deliver specialist SEND guidance, emotional support and advice (under the direction of the Clinical Lead) to children and young people with additional needs and their families supporting parents and carers through the process of diagnosis, discussing developmental concerns sleep behaviour, diet and continence. Signposting and referral to relevant services such as earlybird training, local support groups and parent forums. Working in partnership with Health Education, Social services and Voluntary services to provide a holistic approach in communicating and sharing advice, information and plans to all relevant parties and familyYou will be working as part of a well -established multi-disciplinary team and have the ability to meet the demands of the rapidly changing primary care scene. You must have worked within a community healthcare environment.

NNEB Diploma / BTEC National in Nursery Nursing/ NVQ in Health, Care, or Early years level 3 or equivalent GSCE Maths and English Child Protection/Safeguarding Training Foundation degree in Health or Early years (desirable) Are experienced in working in a health setting IT skilled Can demonstrate an understanding of current issues affecting community health services

Principal Responsibilities
Supporting families with children (0-19yrs) with social communication difficulties Supporting children and families where the child has a significant developmental or complex health need Knowledge and understanding of child health and development to support parenting on a one-to-one basis Delivery of courses to parents in health issues such as sleep, toilet, diet and behaviour, with an understanding of additional needs of SEND children and their families Ability to manage conflict if working with families where challenges or disagreement may occur Experience of building strong partnerships, maintaining relationships and co-producing solutions to challenges across a diverse set of stakeholders, including children, young people and their families. Advocating the voices of those we are working with. Experience of keeping and maintaining records (in accordance with local policy) and producing accurate reports, presentations, written updates to a range of stakeholders (which may include families, children or young people). Knowledge of statutory legislation (for example Childrens Act) and national policy drivers that will affect childrens services and your role To undertake home visits under the guidance of a Health Visitor or Clinical Lead and to provide input to selected families as per care plans To have an understanding of health promotion and be able to run/participate in health promotion sessions and events and organise and maintain health promotion displays. To be aware of local resources and be able to signpost to different agencies To be able to identify child safeguarding concerns and take appropriate action as per child safeguarding guidelines To contribute as part of a team for early identification of need and apply agreed interventions To participate in child protection case conferences and case review meetings as appropriate To have a willingness to learn new skills and adapt positively to changes in working practices. Create, maintain and enhance effective working relationships within the team and with other staff and other agencies in order to promote interdisciplinary team working across the Organisation, Local Health Economy, Social Services and other agencies. To work collaboratively within the Children and Families Team to influence and implement policy and changes in practice.
