Job summary

The role will be key in supporting patients to tackle the issues underlying their ill health, money issues, health issues, social issues and much role will also support proactive management of frailty, to enable them to live healthy, independent lives for longer and to avoid hospital will work closely with allied community health and social care workers, GPs and community groups to support service integration work around mental health, social care, community nursing and the voluntary well as working closely with Therapists, ACPs, Complex Care Nurses, GPs Primary Care, Health and Social care co-ordinators, Well- being centre staff and other services, within our new ageing well clinic

Main duties of the job

Take referrals from GP practices and multi-disciplinary teams, and working closely with clinicians for the benefit of the local population.Support in the delivery of proactive care and preventing deterioration of people living with frailty.Strengthen Community resilience, focusing on what makes a difference to the wider community taking a holistic approach to support the community infrastructureCo-produce a simple personalized care and support plan to improve health & wellbeing introducing or reconnecting people to community groups and statutory and prioritise own caseload, in accordance with the needsHave a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when the persons needs are beyond the scope of the link worker role when there is a mental health need requiring a qualified practitioner.Identify new, and work in partnership with voluntary and statutory organisations.Understand the barriers and opportunities for people to self-manage their conditions in the a role in educating clinical and non-clinical staff on what other services are available within the community and how and when people can access them.

Qualifications

Essential

Level 4 qualification ( certificate of higher education, higher national certificate, NVQ level 4) or equivalent demonstrable experience Functional Skills at Level 2 (Literacy and numeracy/ GCSE grade A-C or equivalent in English and Maths)

Desirable

Mentorship / Coaching qualification

Experience

Minimum of 2 years' experience as a Senior Healthcare worker Experience of working as part of a team Experience with the use of Microsoft Office and a variety of IT packages (E-Pay / E-roster / AT-P / CIS
