Job summary

Worcester City Primary Care Network are looking for a salaried GP for 3 sessions to join our amazing Care Home Team.

£11,000 per session, the 3 sessions ( hours) can be split in different combinations but must cover both Monday and Tuesday. Negotiable start and finish times.

We are a large (approx. 130,000 patients) primary care network set in the heart of the beautiful city of Worcester. The Care Home team is made up of GPs, prescribing ANPs, paramedics, clinical pharmacists, a physicians associate and care coordinator.

The Care Home Team cover the residents within Worcesters Care Homes Monday-Friday.

Main duties of the job

Duties would include ward rounds, supporting and supervising the care home team staff, and speaking to relatives and associated care home patient administration.

The team is based at Henwick Halt Medical Centre as an MDT site.

Job description

Job responsibilities

Conduct reactive and proactive consultations, triage concerns from care home managers, prescribe medications and manage test results.Mentor and supervise clinicians on specific pathways and create learning opportunities through shadowing and training sessions.Lead MDT and Palliative Care meetings to discuss complex patient cases.Perform regular GP ward rounds in care homes, working closely with assigned healthcare professionals.Serve as the duty doctor when on site, making autonomous clinical decisions.Maintain clear and comprehensive IT-based consultation notes and collect data for audits.Prescribe medications in accordance with guidelines and undertake normal GP duties with a focus on frailty.Adhere to practice/PCN policies on prescribing, confidentiality, data protection, and health and safety.Participate in quality improvement activities, including audit and clinical governance. Qualified General Practitioner GMC Registration Registered on the National Medical Performers List Membership of a recognized Medical Defence organisation Evidence of continuing professional development Strong understanding of patient-centred care approaches Effective time management and organizational skills Excellent communication and leadership abilities English language proficiency and eligibility to practice in the UK independently Enhanced DBS check

Experience with local ICB initiatives and policies Familiarity with EMIS clinical system Knowledge of national and local healthcare policies, including QoF Car driver with a full and clean license
