JobSummary:

This post isdefined by the Children (Leaving Care) Act 2000 and attendantguidance andregulation.

The post holderwill support and/or lead the assessment of needs for our EligibleRelevant and Former Relevant young people to determine the level ofadvice assistance and support in their transition to independenceensuring they are able to maximise their lifechances.

KeyDuties/Accountabilities:

To assess theneeds of young people to determine the level of advice support andassistance they will require in making the transition toindependence.

To beresponsible for the formulation coordination implementation andreview of Pathway Plans which will identify how these needs will bemet to maximise lifechances.

To supportchildren in care in their preparation for independent livingincluding supporting with Pathway Planning education andaccommodationneeds.

To provideadvice guidance and practical and emotional support to careexperienced young people individually and ingroups.

Supportaccommodation providers and provide them with advice guidance andpractical assistance required to enable them to provide goodquality accommodation for youngpeople.

To facilitatethe access to appropriate departmental resources and the resourcesof other agencies partners and stakeholders to ensure plans arefullyimplemented.

EssentialExperienceRequired:

Experience ofdifferent approaches methodologies models and direct work toolswhen working with children and youngpeople.

Experience inundertaking risk assessments and safety planning for children youngpeople and/or vulnerableadults.

Directexperiences and/or knowledge of the options for post schooleducation andtraining.

At least twoyears working with adolescents individually and/or ingroups.
