Evaluate the needs of young people to determine the level of advice, support, and assistance required for their transition to independence.
Pathway Plans:

Formulate, coordinate, implement, and review Pathway Plans to maximize life chances for young people by meeting their identified needs.
Preparation for Independent Living:

Support children in care in preparing for independent living, including assistance with Pathway Planning, education, and accommodation needs.
Advice and Support:

Provide advice, guidance, and practical and emotional support to care-experienced young people individually and in groups.
Accommodation Support:

Support accommodation providers by offering advice, guidance, and practical assistance to ensure the provision of quality accommodation for young people.

Resource Access:

Facilitate access to departmental resources and resources from other agencies, partners, and stakeholders to ensure full implementation of plans.

Person Specification:

Qualifications:

NVQ Level 3 in a relevant work area or equivalent (Essential)
Dip SW or Equivalent Diploma in Youth and Community Work or Teaching (Desirable)

Knowledge:

Experience with different approaches, methodologies, models, and direct work tools when working with children and young people (Essential)
Awareness of benefits and entitlements for young people over the age of 16 years and how to access these (Essential)
Knowledge of post-school education and training options for 16-24 year-olds (Essential)
Ability to respond to complex situations in a sensitive, calm, and professional manner (Essential)
Awareness of the Children Act 1989 and Children (Leaving Care) Act 2000 (Desirable)
Understanding of housing issues and experience in supporting people to access and maintain stable and suitable accommodation (Desirable)
Awareness of health needs and issues young people may experience during their transition to adulthood, and ability to support them in accessing health services (Desirable)
Experience in undertaking risk assessments and safety planning for children, young people, and/or vulnerable adults (Desirable)
