Job overview

An exciting opportunity has arisen to join our well established team as a Breast Clinical Nurse Specialist (CNS) in the Jasmine breast unit at DRI and the Princess Diana breast unit at Bassetlaw Hospital.

The role involves offering specialist information and providing psychological support to patients and families who have been diagnosed with breast cancer, through breast screening and symptomatic pathways.

This interesting and challenging role includes working with the MDT to provide advice, holistic care and support to patients attending the breast services.

Candidates should have a high level of independence, excellent communication skills and the ability to work autonomously and as part of the Breast CNS team.

There is also a requirement to work across sites.

Previous experience in the care and management of breast disease or oncology is desirable. Specialised training will be provided by the Breast CNS team in house and external.

Main duties of the job

Leadership- clinical expertise, with high standards of person-centred care

Promotes a culture that supports / enables self-management

Deliver education and training to other professionals involved in patient care

Active member of the multi-professional meeting- patient advocate

Role model with excellent communication skills

Has conflict resolution, negotiation skills when dealing with challenging situations

Support patient focussed education, i.e. train self-manage for ongoing consequences of treatments

Develops clinical, technical and research skills

Demonstrates evaluation of nursing practice and suggests changes

Personal responsibility for life-long learning / personal development through clinical supervision, appraisal and Knowledge and Skills Framework 

Deliver evidence based individualised and personalised care through holistic needs assessment, care planning

Act as an Key Worker for a clinical caseload. Work to proactively manage care and provide easy access for ongoing support for patients

Assesses people's information needs and provide relevant information

Provide clinical expertise, specialist advice and support across service

Follow an agreed care pathway, refer to other professionals and sign-post to other agencies as appropriate.

Promote self-management, risk stratication to determine who can self-manage, those who will need guided support and those who will require on going face to face support, to live with and beyond cancer and its treatment.

If your normal duties are directly or indirectly concerned with patient care you must ensure you receive sufficient training, information and supervision on the measures required to prevent and control risks of infection.

You must be aware of the and adhere to Health and Safety legislation, policies and procedures, to ensure your own safety and that of colleagues, patients, visitors and any other person who may be effected by your actions at work. You are reminded of your duty under the Health & safety at Work Act 1974 to take reasonable care to avoid injury to yourself and others; to officially report all incidents.
