Job Purpose

Play a pivotal role in leading and coordinating clinical nursing practice, education, research, and consultation within our Clinical Community Care division, ensuring high quality evidence based clinical care underpins all actions and interactions with the people we support, in collaboration with multidisciplinary colleagues.

Key Accountabilities & Responsibilities

  • Ensuring applicable clinical standards are met in an efficient and patient focused manner, managing the general standards of infection prevention and control in accordance with company.
  • To contribute to the achievement of the company strategy and objectives by providing clinical and practical advice and support to the business to develop new business opportunities in both existing and new business sectors, participating in presentations, tenders and business proposals as required.
  • Have responsibility for the company’s clinical governance obligations, managing the undertaking of regular clinical quality audits and responding to and ensuring learning takes place with regard to incidents that impact on quality, developing solutions to alleviate risks and managing strategies to address non-compliance.
  • To provide key oversight of clinical risk management and care delivery processes and represent the organisation in a clinical capacity in the management of complaints, serious incidents, local adult safeguarding procedures and undertake clinical investigations where delegated.
  • Collaborate with multidisciplinary teams, healthcare professionals, commissioners, and community partners to deliver outstanding care and support and enhance independence for the people we support.
  • Be physically visible and accessible to the people we support, families and support staff working together to ensure efficient, effective, and caring services, including providing support and guidance out of hours via escalated on call.

  • Ensuring the Community Complex Care Team actively provides excellent levels of support for the client group served, providing clinical assurance that the needs of people supported are met appropriately; identifying likelihood and challenging appropriately and confidently where standards of care fall below that which should be expected.
  • Ensuring there is ongoing assessment, planning, implementation and evaluation of care and support and access to expert knowledge and advice is available to the staff teams.
  • As a role model for others leading the delivery of evidence-based person-centred clinical support
  • Maintaining professional registration and validation with the Nursing and Midwifery Council; ensuring specific education programs are in place to support staff development and competence.
  • Management of direct reports across the Community Complex Care Team ensuring that the goals and direction of the team align with the company; to provide clinical leadership and guidance to the team and the wider business, ensuring the highest standards of care, professional conduct, nursing practice, and staff development. Facilitating and participating in clinical supervision and supervise others as appropriate; ensuring clinical staff are skilled and personally able to offer appropriate support to meet the needs of the people we support.
  • Develop and implement educational programs, workshops, and training sessions for clinicians, ensuring continuous professional development and adherence to best clinical practices.
  • The management of the clinical competency process ensuring this is reviewed, reported and all support workers remain compliant and competent.
  • Utilise advanced clinical knowledge and skills to assess, plan, implement, and evaluate care and nursing procedures, serving as a resource for complex cases; actively raising awareness on relevant Health and Safety guidelines and ensuring arrangements are in place to meet compliance requirements.
  • Engage in evidence-based practice and research initiatives to improve outcomes, enhance care delivery, and contribute to advancements in clinical nursing practice.
  • Ensuring clinical and work-related competencies are maintained for all staff and that multidisciplinary practice is conducted effectively and optimally.
  • Ensuring statutory and mandatory training is accessible and coordinating activity to ensure that all members of the Clinical Community Care team meet these requirements.

Experience

  • Minimum of 5 years nursing experience.

Qualifications & Training

  • Valid nurse registration with NMC, issued with a current PIN number.
  • Professional and recognised nursing qualification.
  • Relevant recent complex care training.
  • Willingness to participate in additional training/development programmes.
