Primary Care Mental Health Specialist Practitioner

Job summary
We have three band 6 posts which are an exciting opportunity for a highly skilled and motivated, mental health nurse, occupational therapist or social worker to be part of the ongoing development of primary care mental health team within Leeds Mental Wellbeing Service.
The primary aim of the team is to support individuals with an unmet mental health need.
The role involves liaising with general practice, the community mental health team, psychology and third sector colleagues to ensure the appropriate service is available to the individual.
Where this cannot be found we ourselves would offer short term intervention which would include formulation, risk assessment and crisis planning, psycho education and coping strategy work.
The post holder will be expected to be able to work both autonomously and as part of a wider MDT but will have robust support and supervision around them.
They will also provide leadership and supervision to their colleagues.
The post holder will have a mixture of working within GP surgeries, community mental health team base and home working.

Main duties of the job
The post holder will provide access to triage, specialist mental health assessment, risk assessment, and brief intervention to prevent deterioration in mental health requiring a referral to secondary mental health services. The post holder will also provide access to triage, assessment and decision support for primary care colleagues and community mental health teams to ensure that service users with complex mental health needs access the correct pathway and intervention to meet their needs.
This will build upon Leeds' vision for the development of a new mental health framework for care in Leeds wrapping specialist mental health clinical knowledge and skills into primary and community settings under the Community Mental Health Transformation process.

Job responsibilities
Ensure effective communication systems are in place and used appropriately. Full participation in appropriate managerial and clinical meetings. Take responsibility for positively engaging service users and carers in all aspects of care delivery and evaluation. Support the development of networks with staff working in other agencies that provide services to service users with complex mental health needs, liaising with a range of staff and clinicians in primary and community health services, social care, independent and third sector. The post holder will report directly to the Band 7 Senior Primary Care Mental Health Liaison Practitioner, and be accountable to the Locality Manager. To provide supervision, support and training to colleagues including through the PDP/appraisal process. Will be able to adapt to barriers in communication that Service Users may present with emotional, psychological and physical, utilising a high level of expertise in communication skills.Recognised professional qualification RMN, RN (Mental Health), OT, Social Worker
Will maintain current specific professional and legal standards of practice in accordance with professional codes of conduct.
Extensive experience of working in partnership with service users, carers and other agencies.
Extensive experience in undertaking specialist mental health and risk assessment, will demonstrate that these skills are maintained and shares those skills with other team members.
Will be able to demonstrate a high level of understanding of the national and local mental health and social care agenda and be able contribute to service development initiatives.
Will be able to demonstrate a working knowledge of clinical, teaching and leadership skills and can demonstrate evidence to support same.
Will be highly skilled in all aspects of clinical communication including the maintenance of accurate records
To demonstrate and apply a highly skilled level of clinical and professional supervision. (Utilising a variety of techniques underpinned by a variety of models and experience). Work actively to reduce mental health inequalities, by applying your knowledge of social inequality of mental health in Leeds and how some communities are at greater risk of poor mental healthTo provide advice, support and information to service users and carers.
To support the development of links with partner agencies
To provide supervision where appropriate to more junior staff and students.
To offer consultation, advice and decision support to primary care colleagues.
To use expert judgement and analytical skills in all areas of clinical practice and decision making, in line with Professional codes of practice
