Job summary

We are currently looking to recruit an enthusiastic and motivated Support Worker to join our Adult Locality Mental Health Team based in Peterborough.

You will be working as a part of a supportive multidisciplinary team that provides services to people who experience significant mental health challenges. You will be one of 7 Support Workers within two Peterborough Locality Teams. As a Support Worker in the team, you will provide a range of practical support to people using the service, within a community setting.

You will work closely with the Occupational Therapists to deliver a Mental Health Reablement Service - adopting an enabling approach to help people achieve their recovery goals, promote independence and help people to develop or maintain their skills in daily living. The interventions offered will be time limited and focused on the service user need.

The ideal candidate will have a strong team work ethic, ability to work independently in different community settings, have a sense of accountability, accompanied by an ability to work under pressure at times.

In return we will provide a team environment that provides the necessary support to ensure your development within your role. We are keen to develop our staff and encourage learning opportunities within the Trust. You will receive clinical supervision and be part of a Trust wide training forum for all mental health Reablement staff, as well as many other learning opportunities within the team and wider Trust.

Main duties of the job

The post holder will provide quality evidence-based support, and care for service users with mental and physical health needs and to their carers within a multidisciplinary setting.

They will be responsible for the supporting of patients, empowering them to be recovery focused and ready to manage themselves along their care pathway, and will have a working knowledge of local, national policies and legislation which govern current service provision.

The post holder will be responsible for identifying and contributing to the training needs of staff and ensure that issues of equality, diversity and rights are dealt with in accordance with good practice and legislation. They will ensure that effective communication pathways exist, and that they act within their sphere of responsibility.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a cannot usepublic transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. To carry out tasks as directed by senior staff in the delivery of high quality care and support to patients and their carers, using a person centred approach in the promotion of their well-being and recovery.
