An exciting opportunity has arisen for a community Nurse to join the Leeds intensive Support Team on a 12-month, fixed term contract to cover a period of maternity leave.
The Intensive Support Team for people with learning disabilities works with people who at a significant time in their lives when they need additional support due to mental health or behavioural need. Many of our service users have experienced emotional and physical trauma, they may have personality disorders, mental health needs, as well as a level of learning disability. In addition, they may be accommodated in services who struggle to meet their challenging needs. The team's overall aim is to keep people out of hospital. We aim to keep people safe, empower service users to overcome individual challenges and provide them and their staff team with the skills and knowledge needed to overcome the present challenges they face. The team is highly skilled, passionate, forward thinking and completely personal centred.
We would like to recruit a confident, dynamic and enthusiastic Nurse to join us for a 12 months period to provide our service users with vital intensive support that people need at this critical time in their lives. We would accept the right candidate with a learning disability or mental health nursing qualification.
Main duties of the job
To work as a valued member of the Multidisciplinary Team To work collaboratively within professionals working with individuals on the IST caseload. Using evidence based/client centred principles. The Intensive Support nurse will assess, plan, implement care to meet the behavioural and mental needs of people at the critical time in their lives, aiming to keep them out of hospital and living in their current environment. Lead in developing and maintaining the delivery of nursing care to promote health and wellbeing. Maintain clear links with your profession's structures and your professional's lead within the directorate and the Trust. Support the professions development and modernisation and adherence to your professional standards and code of practice.To keep up to date with clinical practice through MDT tri-weekly huddles, clinical supervision, occupational health practice development meetings and Allied health professional forums.

Job responsibilities
To ensure effective communication and collaboration with others involved in a service users care.
Liaise as necessary and attend CPA meetings, CTRs, risk meetings reviews and team meetings.
To ensure the person referring and others involved in a service users care are informed and kept aware of involvement both verbally and in writing, with the service users consent.
To ensure the maintenance of a high standard of care to Learning Disability with specialist health needs, supporting the implementation of the Care Programme Approach
To manage time effectively a work with clinicians with the IST caseload.
Seek managerial supervision with Clinical Team Manager (CTM) effectively and efficiently.
To participate in managing and prioritising time effectively.
To maintain links with primary care and advise on mental health issues and support as necessary
To demonstrate sound clinical skills and ability to make decisions, whilst always aiming to promote the involvement of the service user.
To embrace any health promotion initiatives and always encourage a positive attitude to learning disability difficulties.
To maintain clear links with your professions structures and your professions lead within the Trust.
Support the professions development and modernisation and the adherence to your professions standards and codes of practice.
To gain consent and build a collaborative relationship with service users that will promote engagement in the therapeutic process, adapting to barriers in communication which service users may present with emotional, psychological, physical, and always ensuring they are involved in and aware of their current care plan.

The band 6 community nurse will provide direct clinical care to adults with a learning disability with the aim to improve service users lives and prevent admissions to specialist mental health and general health hospitals by ensuring that the right plans are in place.
The post holder will provide clinical supervision to junior colleagues within the team, developing intervention toolkits for nurses as well as holding a caseload including care co-ordination to ensure care teams or family carers are supported to implement care and treatment as recommended.
The Learning Disability service has clear clinical governance pathways and supports professional and service development. As a band 6 there is an expectation to contribute or lead on service development, service projects and task groups. Robust supervision structures are in place with 1:1s with support from the Specialist Nurse Practitioner, peer support through weekly MDT meetings and community nurse meetings.
The successful candidate must have worked with people with a learning disability previously either in the community or in an inpatient setting, be keen to continue to work with people who have complex needs and have awareness of the needs of people who have a learning disability.
The successful candidate will be based within the Intensive Support Team. The service currently runs 7 days a week.
