Anexciting opportunity has arisen for a Trainee Nursing Associate (TNA) to support the Burntwood Primary Care Network.

Main duties of the job

The TNA willwork as part of a multidisciplinary team, providing safe, person-centred careto improve the health of our population. In line with best practice and withsupport from the MDT, you will carry out various procedures to monitor andimprove the health of patients, including physical observation, venepuncture,ECG recording, injections, annual reviews and other procedures commensuratewith your training and skills. Working in collaboration with a range of otherprofessionals including GPs, practice nurses, district nurses, and othercommunity services. Either in practice or attendance at housebound patientsresidence.

A designated senior medical person willprovide mentorship and supervision. The level and type of supervision will bedependent on the post holders acquisition of skills and knowledge anddetermined by the organisations clinical governance arrangements. The practicehold regular education meetings and the TNA would be expected to attend andpartake in these.

Job responsibilities

As a TNA, you willwork under the supervision of and in partnership with the Senior Nurses toprovide and deliver a high standard of nursing care. You will contribute to theongoing assessment, providing and monitoring care based on evidence and workingin partnership with the multi-disciplinary team.

Key responsibilities

Provide compassionate, safe and effective care and supportto patients.

Monitor the condition and health needs of patients withintheir care on a continual basis.

In partnership with patients, families and carers,ensuring appropriate escalation to a registered nurse or other professionalwhen required,

Support patients to improve and maintain their mental,physical and behavioural health and well-being,

Promote comfort and well-being by ensuring that patientspersonal and social needs are met and be able to care for a patients nursingneeds,

Perform and record clinical observations such as bloodpressure, temperature, respiration and pulse.

To safely undertake the following procedures to meet thepatients nursing needs to include undertaking routine procedures includingvenepuncture, ECG recording, recording and monitoring vital signs, wound care,nutrition, hydration, foot care, skin integrity,

Carry out immunisations in accordance with local andnational PSDs (once additional training has been competed),

Provide general and specific health screening referring tothe General Practitioner, when necessary,

Identify and report changes in a patients condition to aregistered nurse or other professional,

Prioritise and manage own workload, recognising whereelements of care can safely be delegated to other colleagues, carers and familymembers,

Provide and receive sensitive information concerning patientsmedical conditions,

Work with registered nurses and other professionals andengage with other agencies and community teams to ensure a safe, personalapproach for patients,

Maintain accurate, contemporaneous patient records in linewith practice policy and the NMC Code,

Provide effective evidence-based care for service usersunder the supervision of registered nurses and other professionals in order toperform non-complex activities, in line with clinical governance andprofessional standards,

Support registered nurses to enable them to be able tofocus on the more complex clinical care,

Care for all practice patients including individuals withdementia, mental health conditions, and learning disabilities.

May include domiciliary visits.

Working towards the completion of the Nursing Associate Apprenticeship programme Enrolled on a foundation degree awarded by a NMC approved provider over a 2-year period. GCSE Grade 9 to 4 (A to C) in Maths & English or functional skills level 2 Maths and English

Knowledge and skills

Essential

Knowledge of Clinical Governance within Primary Care Understanding of the scope of the role of the Trainee Nursing Associate in context of the nursing and interdisciplinary team and the organisation, and how the role may contribute to service development Understands and acts in line with NMC professional standards for practice contained within The Code. Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice Understand requirements for NMC professional revalidation Understanding of the importance of following procedures and treatment plans Ability to take part in reflective practice Ability to organise and prioritise own delegated workload Ability to deal with non-routine and unpredictable Ability to communicate effectively (written, verbal and non-verbal communication) with patients/relatives and carers and all members of the multi-disciplinary team. Ability to develop effective and appropriate relationships with people, their families, carers and colleagues Ability to support, supervise, assess and act as a role model to other learners and health care support workers as required within the clinical setting.
