Our community inpatient rehabilitation ward at Petersfield Community Hospital has an exciting position available for Registered Nurses who wish to take that next step in their career and who have an interest and passion for caring for patients with complex needs requiring a period of rehabilitation or end of life care.

Our wards admit patients directly from their homes or from acute care providers and have close links with our community colleagues.

Our small and friendly team prides itself on the excellent multidisciplinary team approach led by our Consultant Nurse. If you are an enthusiastic, motivated and well organised band 6, or if you are an experienced band 5 looking to take the next step in your career, this could be the job for you.

The successful candidate will have excellent communication skills, be able to provide a high standard of care and become part of a small friendly committed team delivering excellent patient centred care.

Main duties of the job

To be responsible for co-ordinating, delivering and maintaining a high standard of day to day clinical care in all patients in the ward area.

To provide high standards of care for patients and their families by assessing, planning, implementing and evaluating care without direct supervision but within scope of professional practice.

To work as part of an integrated multi-disciplinary team visiting patients providing rehabilitation and health care in a community hospital environment to support their timely discharge from hospital.

Non clinically the individual will support other team members to develop their clinical knowledge. Participate and support with team development to ensure a quality service is provided.

To provide support to the ward manager (Band 7) and clinical leadership to the ward team and deputise for the Band 7 in their absence.
