Job summary

We are looking for a proactive, positive, supportive and reliable qualified Practice Nurse to join us and our current Nursing team. We are open to interest from newly registered Band 5 Practice Nurses (conversion to General Practcie Nursing avialablle if necessary), or Specialist Band 6 Practice Nurses with particular interest in Chronic Disease Management.

This is an exciting time for us in general practice and as we continue to improve and adapt we have some exciting opportunities ahead.

Main duties of the job

The role includes all aspects of the clinical Nurse in relation to wound care, cervical cytology, vaccinations and immunisations and more. Specialist nurses extra responsibilities include long term condition reviews, in particular Respiratory.

To be considered for this role you will need to be a Registered Adult Nurse (RGN) but as a Training Practice we are willing to consider applications from all suitable applicants depending on ability.

Our list size is 37,500 patients. We have a team of more than 100 staff which includes 8 GP partners, 10 salaried GPs, 1 nurse practitioner partner, 1 managing partner and multiple allied health practitioners.

SystmOne is used for patient notes. This is shared across community healthcare teams throughout the city improving communications and increasing patient safety.

We provide a Clinical Assessment Service similar to telephone triage for paramedics, A&E attendees and 111 callers eligible to be assessed by a GP. We can prescribe or book appointments for patients at their own surgery and if needed dispatch an ambulance.

We provide a wide range of in-house services including LARC, joint injections, occupational health, medicals and acupuncture.

We are an established training practice hosting FY2's, GP VTS, paramedic and nurse practitioner trainees as well as medical students with strong links to the new Portsmouth Medical School.

Job description

Job responsibilities

To provide nursingservices to a defined practice population with the assistance of, and incollaboration with the Practice Nursing Team and GP colleagues. This will include clinical duties, healthpromotion, health screening. To contribute in the development of servicesprovided by the Practice and to participate in clinical audit. To work with theGPs and the Specialist Practice Nurses to develop and implement the PracticeDevelopment Plan, the National Service Framework targets and to meet andmaintain the quality indicators contained within the new General MedicalServices (nGMS) Contracts Quality and Outcomes Framework.

KEY RESPONSIBILITIES

Clinical

1. Towork with the Specialist Practice Nurse to assess, plan and provide health careand nursing services to the practice population ensuring they are effective,relevant, evidence-based and of a high standard.

2. Towork with the Specialist Practice Nurse to provide a comprehensive generalpractice nursing service, to include venepuncture (taking blood), wound care,diagnostic blood and urine tests, ear care and advice, new patient registrationhealth checks, over 75year health assessments and travel immunisations andadvice, within the surgery, and where appropriate in patient homes.

3. Towork with the Specialist Practice Nurse and GP colleagues to achieve andmaintain breast and cervical screening targets. To record and monitor results, includingefficient and accurate call and recall systems.

4. Towork with the Specialist Practice Nurse to provide an effective healthpromotion service, to include smoking cessation and healthy lifestyle clinics.

5. Towork with the Immunisation Lead, to include the administration of thechildrens immunisation programme according to current schedules, the accuraterecording of the information in the child health records

6. Towork with the practice team in order to administer the annual influenza andpneumococcal vaccination programme. To include the identification, targetingand immunisation of vulnerable patients within the practice population, toensure that local and national targets are met.

7. Toensure that professional responsibilities are met as a practitioner supplyingand administering medicines in accordance with a Patient Group Direction.

8. Toassess patient understanding of treatment proposals, gain valid informedconsent and works within the NMC Code of Professional Conduct to provide carefor patients who lack the capacity to consent to treatment.

9. Towork with the practice team to ensure clinical governance standards are met,including managing clinical risk within own clinics, active participation inclinical risk management meetings, and dealing with complaints.

10. Towork closely with the practice team, primary care and community nurses, alliedhealth professionals and social services colleagues by active participation inclinical meetings, to ensure effective lines of communication in order toachieve a high standard and continuity of care for the practice population.

11. Toassist in emergency cleaning of bodily fluids when requested in line with thePractice Infection Control policy
