Job summary
Magdalen Medical Practice is looking to welcome a qualified and experienced clinical pharmacist practitioner into our busy practice, to play a full and active role within our multi-disciplinary team, on a full time basis for 37 hours per week.
Main duties of the job
Clinically assess and treat patients, using expert knowledge of medicines, for specific disease areas. Reduce inappropriate polypharmacy and wasteful prescribing through clinical medication review. Reconcile medicines following hospital discharge & work with patients & community pharmacists to ensure patients receive the medicines they need post discharge. Contribute to reductions in medicine related hospital admissions/readmissions by supporting patients to get the best outcomes from their medicines & identifying and addressing medicines related issues. Interface with community & hospital pharmacy colleagues to develop referral processes between primary care professionals including the promotion of the repeat dispensing service. Manage the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions & medicines reaching review dates. Manage patients & health care professionals medicine queries. Increase quality & safety of prescribing through mechanisms such as audit and PDSA cycles. Act as a lead for the prescription hub team, supporting them where necessary. Liaise & remain engaged with the Norwich North PCN pharmacy team members as a whole.

Qualifications
Essential
Completion of a degree in pharmacy and registration with the General Pharmaceutical Council. Qualified from, an approved 18-month training pathway or equivalent that equips the clinical pharmacist to be able to practice and prescribe safely and effectively in a primary care setting (for example, the CPPE Clinical Pharmacist training pathways). Post-qualification experience in pharmacy (hospital, primary care or community), or working towards obtaining this. Ideally be a qualified independent prescriber, or working towards this.

Essential
Experience of prescribing and undertaking medication reviews. Awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice. Ability to demonstrate general pharmaceutical skills and knowledge in core areas. Ability to plan, manage, monitor, advise and review general care plans for patients with long-term conditions; Ability to demonstrate accountability for delivering professional expertise and direct service provision as an individuals. Ability to recognise priorities when problem-solving and identify deviations from normal patterns, referring to seniors or GPs with confidence, when appropriate. Non-judgemental attitude and ability to be flexible and adaptable. Self-motivated with excellent communication skills. Understand the need for patient confidentiality.-
