Job summary

Rural West Primary Care Network (PCN) and Southern Health NHS Foundation Trust are excited to be jointly recruiting a Mental Health Practitioner work in our PCN's Mental Health Team. We welcome applications from any registered professional background where you have an interest in working within mental health services and have current UK registration.

Rural West PCN comprises of 2 practices in rural north Hampshire, providing high quality and personalised care for patients, Tadley Medical Partnership and Watership Down Health Oakley. Primary Care offers an excellent work life balance with Monday to Friday working patterns and no weekend or bank holiday work.

Main duties of the job

To work with Primary Care Network to support and help deliver the transformation work around "No Wrong Door To support the planning and coordination of service users' care in primary care including those on the SMI list liaising with teams and agencies as required. To work on signposting to appropriate agencies for ongoing support. To discuss primary care referrals that require referral to secondary mental health care prior to making a referral To be able to deliver therapeutic interventions to service users whose immediate needs may need help in a primary care setting.

The transformation will happen in staggered stages; Hampshire CAMHS, part of Sussex Partnership NHS Foundation Trust joined Southern Health in February 2024, with Isle of Wight NHS Trust mental health and community services transitioning in early May 2024. The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.

Qualifications

Essential

Qualified as a registered mental health professional with current UK registrationAdvanced computer skillsAbility to fulfil travel requirements of the post

Desirable

Completion of in-depth safeguarding trainingAdditional qualifications in psychosocial interventionsMentorship course/ evidence of CPDExperience of working with clients with severe and enduring mental health problems.Clinical Supervision Training
