Job summary

An exciting opportunity has arisen in our Merton Single Point of Access-Primary Care Liaison Service (SPA-PCLS). We are looking to recruit a highly motivated Senior Nurse Practitioner that will beresponsible for supporting the Team Manager in leading the Merton SPA-PCLS.

This is a valuable opportunity for an experienced, enthusiastic practitioner wishing to develop their skills within the Adult Community Service Line.

Main duties of the job

The postholder will be part responsible for establishing and developing the newly transformed Merton SPA-PCLS. This will require working across the organisation to bring about improvement in the care and treatment of service users experiencing mental health issues and their families. They will support the development of the integrated service that functions as the front door for mental health patients accessing the integrated recovery hub through triage, assessment and signposting. The primary aim being to assess and maintain patients' physical and mental wellbeing. The post holder will also have an important role in the supervision, training and education of the wider team.

The post holder will carry an assessment caseload and be responsible for the management and planning of those service users, under the supervision of the Team Manager and Consultant Psychiatrist, including discharge to alternative primary care services, or onward referral to secondary care or specialist teams within the trust.

The post holder will be required to work autonomously, and robust assessment/formulating skills are essential. Experience in crisis management and an ability to work flexibly will be an advantage given the high volume of referrals.

Job responsibilities

To be responsible for carrying out robust specialist assessments ensuring discharge from the service is clinically sound and timely whilst escalate for further clinical advice when clinically appropriate.

To be responsible for making clear autonomous decisions based on an analysis of complex presenting problems and judgement about available options which will include those related to the admission and discharge of patients.

To provide specialist consultation and support for Band 6 practitioners, assisting them in decision making in difficult and complex cases.

To support with the triage of complex calls and escalation whilst carrying out the duty function.

To coordinate resources across the whole service in order to meet more pressing needs and step down into other functions when clinically and operational required.

To work with colleagues to develop and promote the services philosophy, framework of care delivery and strategic objectives, taking responsibility for specific areas of policy and strategy delivery.

To work with colleagues to develop clinical expertise and high standards of nursing practice, as defined by the Nursing and Midwifery code of professional conduct (2015).

To deliver care that is evidence based, reflecting current best practice and research.

To participate in structured clinical supervision in accordance with Trust policies and procedures and to take responsibility for the supervision of nurses and others in non mental health settings working with patients with mental health problems.

To demonstrate clinical excellence and high standards of nursing practice to other staff.

To demonstrate sound understanding of the legal and ethical issues in caring for the mentally ill. In particular, demonstrating sound understanding of the Mental Health Act (1983)/Mental Capacity Act and its use in a non mental health setting.

To provide clients and carers with verbal and written up to date information on the services offered by the Trust, how to express their views, complaints procedure and what they can expect from the service.

To provide an effective link between the specialist community mental health teams and general hospital services.

To provide highly specialist knowledge and expert advice to health and social care professionals providing specialist support and intervention to patients, families and carers.

To communicate clearly and sensitively in situations that are contentious and/or hostile, demonstrating a high level of interpersonal skills, self awareness and empathy in all communications.

To negotiate and problem solve in contentious and/or antagonistic situations where agreement is required or communicate clear decisions and rationale when agreement cannot be reached in situations where the Mental Health Act may be needed to be used.

Person Specification

Training & Qualifications

Essential

RMN (current NMC registration) Teaching, Mentor or assessing qualification. On-going evidence of continuing professional development. Current and full UK drivers license

Desirable

Management or project management qualifications. First Degree Educated to Master's degree Level or equivalent diplomas supplemented by specialist training, experience, short courses that constitute a Master's level or equivalent.

Experience

Essential

Clinical management and leadership experience. Evidence of working with primary and secondary care, local authorities and voluntary organisations. Experience in communicating sensitive and challenging information. Clinical knowledge base in order to ensure scrutiny of compliance with statutory guidance and best practice. Experience of working with adults who have experienced serious ongoing mental health problems. Experience of recruitment and selection of staff. Experience of managing and monitoring budgets. Experience of working as a Clinical Supervisor. Experience of implementing and monitoring standards of care. Experience of working within a multidisciplinary team. Experience of implementing the principles and practice of the Recovery approach.

Desirable

Demonstrable experience of working at Band 7. non medical prescriber or demonstrates academic and clinical competence to undertake training to become a non medical prescriber. Experience of change management and implementation of national policy. Experience of managing non clinical staff and other professionals. Experience of implementing significant changes in working practices. Experience of using and supervising others in the use of psychosocial interventions. Experience of research, audit and service evaluation.
