Main duties of the job

You will triage and treat patients wishing to see a health careprofessional, making any necessary referrals to other members of the primary healthcare team.

Job Summary

The post-holderwill be expert in the delivery of primary care to patients in the practicepopulation (with the only exception being children under the age of one),having direct clinical involvement and extensive clinical knowledge.

To assess, diagnose, treat, refer or signpostpatients who attend surgery with undifferentiated or undiagnosed conditions.

They will work atan independent and autonomous Care Practitioner level providing a first pointof access in primary health care

To undertake home visits, includingNursing/Residential Homes primarily for acute conditions, as assessed by yourduty doctor, and within the boundaries of professional qualifications.

The post holder will use advanced clinicalskills to provide education to patients, promoting self-care and empower themto make informed choices about their treatment.

They will be ableto provide clinical supervision and support to the nursing team, advice andsupport while liaising with other practice staff.

To enableclinicians to safely travel to visit patients in their own home; they areobliged to ensure that your vehicle is in good working order and covered bybusiness class car insurance (any additional premium will be paid by thepractice upon receipt of a copy of the vehicle insurance cost)

Uponrequest, a percentage of the Nurse Practitioners time may be spent on personaland practice development activities including audit, research, team developmentand clinical governance.

JobResponsibilities:

Diagnosing and treating patients

Triage and treat patientswishing to see a health care professional, making any necessary referrals toother members of the primary health care team

Acute care providing clinical support/dealing with patients bookedinto the GP On Call clinics

Domiciliary Care seeing patients in their own homes who are too ill toattend surgery and/or performing a ward round/virtual ward round at ourregistered Nursing Homes

Patients with problems needing referral tosecondary care should be discussed with the Usual GP before making suchreferral.

Ensure clinical Practice is safeand effective and remains within boundaries of competence, and to acknowledgedlimitations.

Advise patients on generalhealth care and minor ailments, with referral to other members of the primaryand secondary health care team as necessary

To undertake home visits,primarily for acute conditions, as assessed by the Duty Team, and within theboundaries of professional qualifications.

Pathological specimens and investigatory procedures

Training and personal development

Training requirements will bemonitored by yearly appraisal and will be in accordance with Practicerequirements. Personal development will be encouraged and supported by the Practice.It is the individuals responsibility to remain up to date with recentdevelopments.

Participate in the education andtraining of students of all disciplines and the induction of all members of thePractice staff where appropriate

Maintain continued education byattendance at courses and study days as deemed useful or necessary forprofessional it is necessary to expand therole to include additional responsibilities, full training will be given.

The post-holder will participatein any training programme implemented by the Practice as part of this employment.

Liaison

As well as the nursing team,there is a need to work closely with Administration to ensure the smoothrunning of the Practice, reporting any problems encountered to the relevantperson.
