Main duties of the job

Canyou make professionally autonomous decisions?

Wehave a well-established practice team including 4 UCPs, one of whom doeshome visits, 2 Advanced Nurse Practitioners, 1 Nurse Practitioners, 4 Practice Nurses, 1Mental Health Nurse, 2 HCAs, two physios, two clinical pharmacists and a CareCoordinator along with GPs and administrative staff. Job description

Job responsibilities

Topractice as an Urgent care practitioner and demonstrate advanced clinicalcompetence and knowledge above those associated with conventional nursingroles.

Provide a point ofcontact within the Practice for patients presenting with undifferentiated,undiagnosed problems, making use of skills in history taking, physical examination,problem-solving and clinical decision-making, to establish a diagnosis and managementplan.

Assess,plan, treat and refer patients appropriately, carrying out consultations in thesurgery or the community as an autonomous practitioner.

Usingown clinical judgement to diagnose and treat patients either face to faceconsultation, clinical telephone triage and other mediums.

Makingappropriate referrals to primary, secondary and social services care, followingpractice (and/or locally agreed) pathways and protocols.

Torecognise the need for and initiate where appropriate relevant diagnosticinvestigations, using their skills in this area to take the correct course ofaction.

Understandinglocal safeguarding referral process and be aware of appropriate referralmechanisms for children and vulnerable adults

Ensurecomplete and accurate documentation of every patient contact, including advicegiven. Records must be accurate and reflect the clinical activity in accordancewith NMC guidelines on clinical record keeping.

Minorillness assessment, diagnosis and treatment with access to seek advice, reviewand onward referral within clinical teams at Cutlers Hill Surgery.

Todevelop care plans for patients when required (as developed and supervised withthe Partners)

Beinvolved with continuity of care of our patients, end of life care,chronic disease management.
