Job summary

The White House Surgery is looking to welcome a dedicated and knowledgeable clinical pharmacist into ourforward-thinking and supportive practice. This exciting role is offered on a part-time permanent basis and will play a full and active role within our multi-disciplinary team

Our clinical pharmacist willplay a vital role within our team, managing medicines-related queries, carryingout structured patient reviews, and contributing to our medicines optimisationagenda. They will work alongside our established clinical team and ourwell-trained administrative team skilled in managing clinical correspondence,data entry, signposting, social prescribing, and medicines management, ourpractice is focused on delivering high-quality care and support for both staffand patients.

Main duties of the job

Being the primary contact for medicine-related queries. Conducting in-depth medicine reviews with patients. Performing structured reviews in care homes. Assessing and treating patients, guiding to healthcare services as required. Working closely with GPs on medication updates and solutions. Collaborating with the dispensary and practice teams to support medicine optimisation goals. Reconcile medicines following discharge from hospitals, intermediate care and into care homes, to ensure patients receive the medicines they need on discharge. Promoting cost-effective prescribing and patient self care. Managing acute and repeat prescriptions. Conducting clinical audits. Implementing medication monitoring systems.

About us

The White House Surgery isa purpose-built medical centre who are looking to recruit a well-motivated andexperienced pharmacist to complement our current team. We are a friendly, busypractice with a list size of just over 7300. We are a high QOF achieving practice with a Good CQC rating. The centre is well led and supported by 4 GP partners longside 3 salaried GPs, an ANP, a paramedic, 3 nurses and 2 HCAs.We employ a team of well-trained admin and receptionists who are led by ourteam leader/reception lead. We are a forward-thinking practice and are involvedin the local community and network of surrounding practices. We are also atraining practice for doctors and students as well as being an active research practice.

Job description

Job responsibilities

Working closely with GPs to provide medicine related updates and helping with problem solving. Provide a point of contact for the practice for all medicines-related queries for healthcare professionals as well as patients. Face to face and telephone mediation review of patients with polypharmacy especially for older people, people in residential care homes and those with multiple co-morbidities. Carrying out structured medicines-optimisation reviews in our care homes. Clinically assessing and treating patients, using expert knowledge of medicines, for specific disease areas. Follow legal, ethical, professional, and organisational policies/procedures and codes of conduct. To support GPs and other practice staff to deliver on Medicines Optimisation agenda (Prescribing Quality Scheme), QOF and locally commissioned enhanced services. To encourage, support and implement adherence to local formulary guidelines and cost-effective prescribing Contributing to repeat prescribing safety prompting patients for reviews, blood tests, monitoring etc. Working with the multidisciplinary team to improve medicines management and safety within the practice including updating, writing, and ensuring compliance with practice policies. Implementing NICE guidance through audit and feedback, formulary management and educational sessions with the wider primary healthcare team and patients. Person Specification

Qualifications

Essential

GPhC registration Pharmacy degree

Desirable

Post-graduate clinical qualification, diploma level or higher. Completion of CPPE 18 months. Prescribing qualification. Experience of working in General practice.

Experience

Essential

Has completed the Pharmacist Foundation programme or a Minimum of 2 years experience as a pharmacist. Evidence of recent CPT and/or other professional development.

Desirable

Experience working within primary care.
