Job summary

Our PCN is looking for a physiotherapist for between 15 and 30 hours per week to join the PCNs wider MDT team. Our PCN has its own building located in South Shoebury and has a 4 practices which the PCN covers. This role will be working autonomously set in your own clinics but will have clinical support from other clinicians in the PCN. The shift times can be flexible with the option of working later in the evenings and Saturdays for enhanced rates of pay.

Main duties of the job

To apply, you should be a fully HCPC and CSP registered physiotherapist with at least 5 years post-graduate experience, including 3 years of relevant MSK clinical experience in line with HEE's Roadmap to Practice.

You should be a driven personality and passionate about developing your clinical skills to become the very best diagnostic clinician you can become.

You should be flexible to the changing fast-paced nature of working in primary care and rise to the challenges it presents.

The applicant should be able to work to a 20 minute appointment timing

Identify patients that need a joint injection and refer to other clinicians if applicable

Work as part of a wider MDT in the PCN to deliver PCN projects such as the CVD LES

About us

Southend East PCN is located at Norton Place in Shoebury, along Ness Road. We have our own PCN building which is extremely active and is in use by our 5 member practices in the local area. We cover approx 37,000 patients and offer a range of appointments from our wide range of staff, fully utilising the ARRS budget. Our team consists of pharmacists, pharmacy technicians, social prescribers, health trainers, paramedics, physiotherapists and assistant practitioners. We have 2 clinical directors who are partners in two local surgeries, and an operations manager to provide support wherever needed. We have a strong digital presence and have employed a Digital Transformational Lead, to help our PCN network with other businesses and mature over time.

Clinical Practice

Assess, diagnose, triage, and manage patients, taking responsibility for the management of a complex caseload. You will progress and request investigations when trained to do so (such as x-rays and blood tests) and referrals to facilitate diagnosis and choice of treatment. Develop and make use of your full scope of clinical practice, including skills relating to independent prescribing, injection therapy and investigation when trained to do so. Manage complex interactions, including working with patients with psychosocial and mental health needs, referring to social prescribing when appropriate. Deliver programmes of supported patient self-management, in ways that facilitate behavioural change, optimise individuals' physical activity, mobility, fulfilment of personal goals and independence, and that minimise the need for pharmacological interventions. Provide care that is tailored to the individual needs. This would include, appraising the impact of individuals' clinical status on their general health, well-being, employment status (including in relation to function, physical activity, mobility and independence). Support patients to set their own goals and be confident in their approach to self-management.

Evidence, Research, and Development

Develop relationships and a collaborative working approach across the PCN supporting the integration of pathways in primary care. Work as part of an MDT in a patient-facing role, using your expert knowledge of MSK issues, to create stronger links for wider MSK services through clinical leadership, teaching and evaluation skills. Support research and audit programmes to evaluate and improve the effectiveness of our FCP service. Streamline the pathway of care by providing a responsive service so that patients receive timely access to care.

Leadership

Lead, manage and contribute to service delivery. Implement all aspects of effective clinical governance for own practice, including undertaking regular audit and evaluation, supervision and training. Communicate effectively and appropriately, with patients and carers, complex and sensitive information regarding diagnosis, pathology, prognosis and treatment choices supporting personalised care.

Facilitating learning

Provide feedback and support to other PCN clinical team members on appropriate referral criteria. Critically reflect on your own practice, and continue to develop your skills.

a. work across the multi-disciplinary team to create and evaluate effective and streamlined clinical pathways and services;

b. provide leadership and support on MSK clinical and service development across the PCN, alongside learning opportunities for the whole multidisciplinary team within primary care;

c. develop relationships and a collaborative working approach across the PCN, supporting the integration of pathways in primary care;

d. encourage collaborative working across the wider health economy and be a key contributor to supporting the development of physiotherapy clinical services across the PCN;

e. liaising with secondary and community care services, and secondary and community MSK services where required, using local social and community interventions as required to support the management of patients within the PCN; and

f. support regional and national research and audit programmes to evaluate and improve the effectiveness of the First Contact Practitioner (FCP) programme. This will include communicating outcomes and integrating findings into own and wider service practice and pathway development.

Person Specification

Qualifications

Essential

Fully HCPC and CSP registered practicing Physiotherapist.

Desirable

Completion of some level 7 MSc learning. Qualified injection therapist and / or independent prescriber

Experience

Essential

Minimum 5 years post-graduate physiotherapy experience. Minimum 3 years of clinical experience of delivering musculoskeletal assessment and treatment.

Desirable

Previous clinical experience of first contact triage clinical work; even if in the private sector. Previous experience of working in a GP practice. Previous experience of using EMIS / SystmOne.

ARRS Requirements

Essential

a. has completed an undergraduate degree in physiotherapy; b. is registered with the Health and Care Professional Council; c. holds the relevant public liability insurance; d. has a Masters Level qualification or the equivalent specialist knowledge, skills and experience; e. can demonstrate working at Level 7 capability in MSK related areas of practice or equivalent (such as advanced assessment diagnosis and treatment); f. can demonstrate ability to operate at an advanced level of practice
