Job summary
We are looking for acandidate who has completed a nationally recognised diabetes educationprogramme at postgraduate diploma level to work in the practice 2 days a week covering 14 hours a week
The role will include the management of diabetes including insulininitiation, health education, reviewing blood results, medication monitoring,and dose adjustments. The idealcandidate will have a passion for diabetes and delivery high quality patientcare.
We are flexible and will work withthe candidate to agree the days worked within the practice to cover 14 hours a week.
Main duties of the job
To be responsible for the managementof diabetes including insulin initiation, health education, reviewing bloods,medication monitoring and dose adjustment.Working as part of the practice multidisciplinary team, delivering carewithin their scope of practice to the entitled patient population. The rolewill include supporting the management team in the reviewing of clinical policyand procedure.
The practice must ensurethat the post holder has access to appropriate clinical supervision.

We are a PMS practice with a list size of 8, fantastic Practice clinical team currently includes 3 Partners, 1 Salaried GP (soon to be 2 Salaried GPs), 2 Practice Nurses and a clinicians are supported by team of excellent admin staff, with roles including receptionist, secretary, data input clerk and a prescription is in addition to an evolving team of Networkemployed clinicians such as pharmacists, pharmacy technicians, Physician Associates, First Contact Practitioners, Paramedics and Mental Health Practitioners

Job responsibilities
Thefollowing are the core responsibilities of the diabetic practice nurse. Theremay be on occasion, a requirement to carry out other tasks; this will bedependent upon factors such as workload and staffing levels:
1.Providea first point of contact within the practice for the management of diabeticpatients
2.Initiateinsulin and proactively monitor the care of all diabetic patients in line withlocal and national policy.
3.Providehome visits for those who are unable to attend the surgery
4.Assessthe needs of patients ensuring the provision of healthcare is appropriate,incorporating evidence-based practice
5.Requestpathology services and process pathology results as required
6.Develop,implement and embed health promotion and well-being programmes
7.Provideroutine nursing care to patients as required in accordance with clinical basedevidence, NICE and the NSF
8.Requestpathology services as necessary
9.Processpathology results as required
10.Deliverpatient care as necessary, referring patients to secondary / specialist care asrequired
11.Maintainaccurate clinical records using ARDENS templates
12.EnsureSNOMED codes are used effectively
13.Maintainchronic disease registers
14.Prioritisehealth issues and intervene appropriately
15.Supportthe team in dealing with clinical emergencies
16.Recognise,assess and refer patients presenting with mental health needs
17.Supportpatients in the use of their prescribed medicines or over the counter medicines(within own scope of practice)
18.Liaisewith external services / agencies to ensure the patient is supportedappropriately (vulnerable patients etc.)
19.Supportthe clinical team with all safeguarding matters, in accordance with local andnational policies
20.Understandpractice and local policies for substance abuse and addictive behaviour,referring patients appropriately
21. Deliver opportunistichealth promotion where appropriate
22. Prescribe medicationfor patients in accordance with PGDs and own scope of practice
23. Review patientmedications to enhance compliance
24.Complywith infection control protocol
