About the Role

Leighton-Linslade Health Connections has an exciting opportunity for a hardworking & enthusiastic Paramedic to join our PCN to work within our Primary Care Network multidisciplinary healthcare team, delivering on the day minor illness clinics. This role is offered on a full-time basis for hours per week. This is an exciting time to join the PCN with new services developing to support our patients & practices.

The main responsibilities of the role include: Urgently respond to referrals & provide assessment, diagnosis & treatment at first point of contact by attending to patients, according to patients’ needs either in the practice or patient homes; Support & continue the development of, an on-the-day emergency patient service in the practice; Accurately triage & prioritise patients, including in both emergency situations & telephone triage, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical & drug history; Evaluate clinical information from examination and history taking, & initiate appropriate treatment &/or referral; Review the effectiveness of the treatment provided, making changes where necessary to improve patient outcomes; Maintain accurate clinical records in an efficient manner; The role will offer you the opportunity to build on your experience of working effectively within a multi-disciplinary, healthcare team, as you will work alongside fellow professionals within the practice. Please see attached for further information. About the Candidate If you are an experienced paramedic wanting a new challenge, we would love to hear from you. You will have the following skills and experience: Be a registered paramedic, with current HCPC registration and have post-registration experience, ideally with some being in primary care; Have completed ENP/ ECP Minor Injuries and Minor Illness course Level 6 and be a ALS/APLS provider;
Be able to effectively assess, plan, treat and refer patients; Be able to evaluate patient care confidently; Have experience of working to protocols and guidelines; To exhibit excellent verbal and non-verbal communication skills; Have the ability to make clear decisions with confidence and communicate these effectively; Understanding of the need to work within the parameters of one's own training and capability, according to the Health and Care Professions Council; Experience of autonomous practice working and multi-professional team working;
Can demonstrate a broad and sound knowledge of complex and common conditions in primary care in both adults and children.

We offer various primary care support roles to help the population take ownership of their health and connect with the community; working with the Personalised Care Model to help patients achieve their goals and focusing on what is important to them.
