About the Role

Ongar Health Centre has an opportunity for a PCN anticipatory care co-ordinator to join our primary care network (PCN). This post will operate within the Epping North Primary Care Network supporting six local GP practices. As part of the PCN’s ambitious plans for transforming and developing primary care locally, the post holder will be expected to operate flexibly and collaboratively with all stakeholders. The role will be part of a multi-disciplinary team and will be central to the delivery of key network objectives.

The post holder will provide capacity and expertise to support patients in preparing for, or following up, clinical care conversations they have with primary care professionals.

The role will require working closely with GPs and other primary care professionals within the PCN to manage a caseload of identified patients, making sure that appropriate support is available to them and their carers, and ensuring that their changing needs are addressed.

The focus of the role will be to support population health management and to reflect local priorities and health inequalities.

The role of the anticipatory care co-ordinator will support the clinical director in coordinating all key activities including access to services, advice, information and ensuring health and care planning is timely. The role will also support digital initiatives and includes supporting the co-ordination of the patient’s journey through primary care.

The ideal applicant will have the following skills, experience and qualifications:

ECDL or equivalent Diploma/HNC Level or relevant experience NVQ Level 2 Business Administration or relevant experience Demonstrable commitment to professional and personal development Advanced experience of using Word, Excel and PowerPoint, including the ability to use word processing skills, emails and the internet to create simple plans and reports Ability to distil information, maintain effective records and prioritise and coordinate actions Creative problem solver, willing to search for hard-to-find information Ability to read large amounts of information and extract the salient points, to analyse data and report on finding Experience of data collection and providing monitoring information to assess service impact Experience of partnership collaborative working  Ability to organise, plan and prioritise on own initiative, including when under pressure and meeting deadlines Able to work effectively as part of a team Able to support people in a way that inspires trust and confidence. Ability to communicate effectively, both verbally and in writing with patients, their families, carers, partner agencies and other stakeholders.  About Us Ongar Health Centre is a large, busy training practice, serving a patient population of 12,000 and operating from purpose built premises.We use SystmOne clinical system and are rated Good with CQC.
