Job summary

The Community Nursing Service within North East Lincolnshireprovide nursing care and support to patients, families and carers in avariety of community settings supporting patientsto remain in their own homes, preventing hospital admission and facilitation ofearly hospital discharges.

TheCommunity Nursing Service works closely and in partnership with Primary CareNetworks, Secondary Care, Social Care, Specialist Nurses and other healthcarestaff to provide a quality service tailored to meet individual needs. Theservice supports and assists patients to manage ill health and disability, thusallowing them to have the best possible quality of life, while maintainingdignity and respect.

Main duties of the job

Health Care Assistant

Pilgrim Primary Care Centre Immingham

Hours: 30 hrs a week - you will be required to work a rota patternwhich includes weekends and Bank Holidays

Salary Range 3£22,566 fixed salary point

You will work on your own in the community,under direction of the team, to provide a range of care needs to our clients, such as wounddressings, eye care, pressure area care, venepuncture, monitoring of blood pressure, weightand blood sugar as well as palliative and terminal care.You will also provide emotional support to patients, carers and family members therefore must have an empathetic and sympathetic approach with effective communicationskills. Most importantly you must beable to demonstrate your commitment to the values of Care Plus Group.

You must hold a relevant qualification, QCFLevel 3 in Health and Social Care. A Level 2 qualification in Numeracy andLiteracy is also essential or an ability to demonstrate an equivalent level ofcompetence. You will have accurate and proven record keeping and ITskills.

In returnyou will have access to further training and development with supervision and support.

You must be able to transport yourself and equipment acrosslocations within North East Lincolnshire to deadlines and you will be required towork evenings, weekends and bank holidays on a rota basis showing a flexibleapproach to working arrangements.
