Job summary

Beckenham PCN is seeking to recruit a Pharmacy Technician todevelop and support medicines optimisation work across the 6 GP practices inthe Beckenham Primary Care Network (PCN).

This is an exciting opportunity for a Pharmacy Technician tojoin a developing Medicines Team within the PCN footprint but also to work inan integrated way alongside NHS South East London CCG Medicines Management ClinicalPharmacists and Technicians. The post will be directly employed by BeckenhamPCN, based in Beckenham, but will work across a number of Beckenham PCN Practicesand will be supported by the well-established CCG Medicines Management Team andour GP practice Clinical Pharmacists who all have a wealth of primary careexperience to support your development.

The post holder will work as part of a multidisciplinaryteam in a patient-facing role. Working alongside other Technician andPharmacist resource within our GP practices and CCG, you will support the GPsurgeries within Beckenham to develop the PCN pharmacy service. You will be enrolled on the18-month national Primary Care Educational Pathway (If applicable).

Main duties of the job

The Pharmacy Technician will need to be committed to improving health and putting patients first. Key qualities will be the ability to build effective relationships with patients, GPs, nurses, community pharmacists, practice(/Hospital) staff, & to work effectively as part of a team.

It is a requirement of the role to be registered with General Pharmaceutical Council. Primary care experience though an advantage, however is not essential. Flexibility, enthusiasm & excellent communication skills are essential.

Applicants will need to be computer literate, confident in independent working, able to prioritise workload & work in a multi-disciplinary environment.

The post holder will:

Support, promote & implement high quality, evidence-based, cost-effective and safe use of medicines. Improve prescribing practices across PCN Undertake patient facing & support roles to ensure effective medicines use. Provide support to GPs, Clinical Pharmacists, other clinical staff and reception teams, coordinating discharge summaries & medication reviews. Play a central role in repeat prescribing processes. Liaise with patients, nursing homes, carers, trusts and community pharmacies, responding to medication queries & updating medical records.

The Pharmacy Technician will undergo clinical supervision & mentorship from Clinical Pharmacists.

Beckenham PCN is a Primary Care Network with GP Shareholdersdeveloped to provide services into the community. We are a group of 6 practicesand we aim to work together to sustain realimprovements in both the care of our patients and our working lives. We as a PCN cover a population ofapproximately 55,000. The 6 practices in the PCN are: Elm House Surgery, KelseyHealth Care, Cornerways surgery, Eden Park Surgery, St James’ Surgery and ManorRoad surgery. The purpose of a PCN is to enable GP practices to work togetherin a collaborative way to develop and deliver network-based services thatrespond to the needs of the local population. Working together Beckenham PCN practices are hoping to develop a highquality, multi-disciplinary workforce that focuses on the needs of thepopulation in our local area.

The successful candidate will join a core Pharmacy Team andwill be supported by a team of Clinical Pharmacists.Regular development sessions, teaching, training and opportunities to upskillwill be provided.

KEY DUTIES AND RESPONSBILITIES

Clinical Duties

To undertake technical medication reviews, or assist pharmacists with clinical medication reviews where applicable.

· To assist practices with primary andsecondary care interface issues.

· Assist patients with medication complianceissues and counsel them about medication inhalers, eye drops etc.

· To support the pharmacists withmedicines reconciliation in accordance with protocols and to update patients’medical records / prescribing accordingly.

· Support medication reviews and medicinesreconciliation for new care home patients and synchronising medicines forpatient transfers between care settings and linking with local communitypharmacists.

· Providespecialist expertise, where competent, to address both the public health andsocial care needs of patients, including lifestyle advice, service information,and help in tackling local health inequalities.

· Take acentral role in the clinical aspects of shared care protocols and liaising withspecialist pharmacists for more complex patients.

· Supportthe timely monitoring and management of high-risk medicines.

· Supportinitiatives for antimicrobial stewardship to reduce inappropriate antibioticprescribing.

· Assist in answering medicine relatedenquiries from GPs, other practice staff and patients in a professional mannerreferring to the clinical pharmacist when appropriate.

· Undertake simple audits of prescribingin areas as directed, feedback the results and implement changes in conjunctionwith the practice team.

· Provide advice to GPs, staff andpatients in changes to prescribing in order to support the improvement ofprescribing safety, quality and cost effectiveness.

Technical and Administrative Duties

Provide training and support on the legal, safe and secure handling of medicines, including the implementation of the Electronic Prescription Service (EPS).

· Support the implementation of the RepeatPrescribing Policy to ensure safe repeat prescribing processes and reducemedication wastage.

· Support the clinicians and practicepharmacists with the implementation of a repeat prescribing reauthorisationprocess.

· Promote Electronic Repeat Dispensing(eRD) and online ordering.

· Help to plan, develop and support theintroduction of new working processes to optimise the quality of prescribing.

· Supportthe PCN/practices to deliver on QIPP agenda, QOF and locally commissionedenhanced services.

· Work with managers, practice pharmacistsand GPs to ensure the practice is compliant with CQC standards where medicinesare involved.

· Carry out housekeeping whilst inpatients records , dose optimisation and medication alignment.

· Advise Clinicians of alternativemedication options when prescribed medication is out of stock or unavailable.

· Provide mentorship and oversight of workfor the Practice Medicines Co-ordinators.

· To deliver information and training to arange of audiences including practice staff.

Person Specification

Qualifications

Essential

BTEC in pharmaceutical sciences / NVQ level 3 or equivalent. Registered with the General Pharmaceutical Council Evidence of continuing professional development

Desirable

Enrolled on or qualified from Primary Care Pharmacy Educational Pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH) or be willing to enrol on.

Experience
