About the Role

The Bridge PCN is looking to recruit an advanced clinical practitioner (ACP) to join our multi-disciplinary team, to help us to transform and modernise pathways of care, enabling the safe and effective sharing of skills across traditional professional boundaries. The role of ACP for The Bridge PCN will be to support our nursing and care home patients. 

The role is part time, ideally working 16 hours per week, though there may be some flexibility in the structure of this over a rolling 4 week period.

The post-holder will need to be an experienced practitioner, who acting within their professional boundaries, will provide care for the patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe clinical decision-making and expert care for patients. They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patient and providing leadership as required. The successful candidate may come from a range of professional backgrounds. As an experienced healthcare professional, educated to master’s level, they will have developed the skills and knowledge to allow them to take on an expanded role and scope of practice caring for patients.

The post-holder will:
demonstrate a high level of expertise within the specialist service providing advice, education and support to staff, patients, their families and carers. be practising autonomously as an advanced clinical practitioner within the designated speciality area to provide patient-centred clinical care. This will encompass the skills of advanced clinical assessment, examination, diagnosis and treatment within an agreed scope of practice throughout the directorate. support new ways of working that emphasises a more efficient and patient-focused service, and will ensure the safe treatment, referral and discharge of patients with undifferentiated and undiagnosed presentations in their area. have responsibility for leading the on-going development of clinical practice and standards of care within the service, including the development of policies, procedures, protocols and guidelines in collaboration with multidisciplinary colleagues. The successful candidate will demonstrate flexibility in attitude and behaviour. They will be keen to undertake new ways of working and delivering healthcare, committing to under-taking the professional development required to adapt to changing circumstances.

Qualifications, skills and experience required:
