Job summary

JOB PURPOSE AND SCOPE:

To work as part of a team that provides secondary mentalhealth services, focusing on the direct needs of service users, working across boundaries ofcare, organisation and provide support and give time to an allocated group of service users in order to promotetheir recovery and maintain them in their community environment helping them to build theirown support network. To support service users to identify their needs and to assist care coordinatorsto plan, implement and evaluate individual care plans. To have the individualservice user's needs at the fore at all times, working to the recovery model to support theservice user to work towards their identified goals. To use agreed values and skills tounderpin day-to-day work. To be actively involved in the d elivery of Initial Interventions withthe support and guidance of a Psychologist or an Occupational Therapist when undertakingSTROT.

Main duties of the job

What we can offer you in our Community Recover Service LineCommunity Recovery offers opportunity to experience community mental health nursing within one of their 9localities across Kent. There is experience available in working with service users and carers within their homesand Group work as part of our MDT working. The service line also has a number of in-patient rehabilitationunits, which will provide newly qualified staff with the opportunity to work with patients as they prepare toleave the in-patient service and move back to the community. There is also the Early Intervention in PsychosisService, which provides a service to 14 - 65 year olds in the early stages of a psychotic illness. Other servicesalso include Learning Disabilities and Primary Care Psychological Therapy Service. Wework in partnership with Kent County Council, which means that staff within thecommunity teams work with Social Workers in providing a high calibre service to bothour service users and their carers.
