Job summary

We are looking for a Band 6 Registered Nurse/ Clinician (HCPC or NMC) to join our Hospice at HomeTeam.

The Hospice at Home Team support patients who arein the last weeks of their life and want to be at home. The team works closelywith families, community nursing teams and the wider hospice services to givethe best care and support possible at this crucial time.

Main duties of the job

Joining the Hospice in this role will involve planning and providing specialist palliative care for our patients and support for their families. The role requires independent working in the community; and as part of the wider health care team.

You will have excellent communication and IT skills and be able to coordinate the service, assess and support patients and families and line manage Band 5 nurses and healthcare assistants to deliver Hospice at Home care.

There is one post available working either hours or 30 hours per week. You will be working hr shifts, 5 days in 7, between 08:00 - 20:00. You will also be contributing to an on call rota.

You must have a clean driving licence and access to a car for all shifts.
