Main duties of the job

The candidate will be expected to provide an exceptional diabetic care service to the patients of the practice.

We are looking for someone who is dynamic and can fit into a small nursing team within the practice.

About us

We are a small practice of 6500+ patients, The staff are all long standing members.

The practice consists of 2 partners, one GP and one Practice Manager Partner. We have 3 other GPs who work at the surgery. We also have a full time paramedic, a full time nurse, a very experienced HCA and a further HCA who is coming along exceptionally well as part of the nursing team.

The admin team offer excellent support to all clinicians and the wider team.

Anyone who joins this team will see how close everyone works and how supportive we all are of each other.

Job Summary

To be responsible for the delivery of nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The nurse will be responsible for a number of clinical areas such as minor illness, health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.

Quality & Continuous Improvement (CI)

Induction Training

On arrival at the practice all personnel are to complete a practice induction programme; this is managed by the Practice Manager.

Learning and Development

The effective use of training and development is fundamental in ensuring that all staff are equipped with the appropriate skills, knowledge, attitude and competences to perform their role. All staff will be required to partake and complete mandatory training as directed by the training coordinator, as well as participating in the practice training programme. Staff will also be permitted (subject to approval) to undertake external training courses which will enhance their knowledge and skills, progress their career and ultimately, enable them to improve processes and service delivery.

Collaborative Working

All staff are to recognise the significance of collaborative working. Teamwork is essential in multidisciplinary environments. Effective communication is essential and all staff must ensure they communicate in a manner which enables the sharing of information in an appropriate manner.

Service Delivery

Staff at Kingstanding Circle Surgery must adhere to the information contained with practice policies and regional directives, ensuring protocols are adhered to at all times. Staff will be given detailed information during the induction process regarding policy and procedure.

Security

The security of the practice is the responsibility of all personnel. Staff must ensure they remain vigilant at all times and report any suspicious activity immediately to their line manager. Under no circumstances are staff to share the codes for the door locks to anyone and are to ensure that restricted areas remain effectively secured.

Professional Conduct

At Kingstanding Circle Surgery, staff are required to dress appropriately for their role. Administrative staff will be provided with a uniform whilst clinical staff must dress in accordance with their role.

Leave

All personnel are entitled to take leave. Line managers are to ensure all of their staff are afforded the opportunity to take a minimum of 25 days leave each year, and should be encouraged to take all of their leave entitlement.

Primary Responsibilities

The following are the core responsibilities of the nurse practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Develop, implement and embed health promotion and well-being programmes

an independent prescriber, prescribe in a safe an effective manner, in accordance with PGD and extant legislation

c. Support the clinical team in the development of individual treatment plans for chronic disease patients

d. Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health

e. Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

f. Provide wound care (ulcer /Doppler etc.) to patients

g. Provide travel medicine services

h. Request pathology services as necessary, processing and interpreting results as required

i. Refer patients directly to other services as appropriate

j. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required

k. Maintain accurate clinical records in conjunction with extant legislation

l. Ensure read codes are used effectively

m. Maintain chronic disease registers

n. Develop, implement and embed well woman clinics

o. Develop, implement and embed well man clinics

p. Chaperone patients where necessary

q. Assist GPs with minor surgery when required

r. Prioritise health issues and intervene appropriately

s. Support the team in dealing with clinical emergencies

t. Recognise, assess and refer patients presenting with mental health needs

u. Recognise, assess and refer patients with community health needs, developing strategies for effective care pathways

v. Implement vaccination programmes for adults and children

w. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

x. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

y. Support the clinical team with all safeguarding matters, in accordance with local and national policies

z. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

aa. Deliver opportunistic health promotion where appropriate

Secondary Responsibilities

In addition to the primary responsibilities, the nurse practitioner may be requested to:

a. Act as the audit lead, effectively utilising the audit cycle

b. Monitor and ensure the safe storage, rotation and disposal of medicaments

c. Contribute to the implementation of an effective training programme for staff and students within the practice

d. Provide clinical supervision for the nursing team

e. Participate in local initiatives to enhance service delivery and patient care

f. Support and participate in shared learning within the practice

g. Continually review clinical practices, responding to national policies and initiatives where appropriate

h. Be aware of and contribute to the planning and implementation of CCG patient pathway initiatives

i. Participate in the review of significant and near-miss events applying a structured approach root cause analysis (RCA).

j. Maintain professional registration and adhere to revalidation requirements
