Bramblehaies Surgery, in Cullompton,is a semi-rural practice in the heart of Devon serving a population of 7,500patients, providing high standards of care (CQC rating of Good) from a partnerowned modern, purpose-built building.

Our team currently consistsof four GP partners, two salaried GPs, Nurse Practitioner, Practice Manager, PatientServices Manager, Data Quality Administrator, Clinical Pharmacist and PharmacyTechnician, a team of three Practice Nurses headed up by a Lead Nurse, threeHCAs, a General Practice Assistant, a committed admin/reception team and our much-neededhousekeeping team.

We are a member practice ofCulm Valley Primary Care Network and are supported with Social Prescribers, Healthand Wellbeing Coach, First Contact Physio, Mental Health Worker, and a Care HomeTeam consisting of a Nurse Practitioner and an Occupational Therapist.

We are a training practice anda Visa-sponsoring practice hosting GP Registrars, and medical students. Our clinicalsystem is SystmOne; we hold a daily clinical huddle for all clinical members ofthe team and we very much encourage clinical development.

The Advanced Paramedic Practitioner will:

g.Supportthe delivery of anticipatory care plans and provide an alternative model tourgent and same day GP home visit for the Practice/PCN.

h.Completethe relevant training to provide multi-professional clinical practice and CPDsupervision to other roles within primary care, for example first contactpractitioners and the personalised care roles.

Essential

Completed a two-year consolidation of learning period when a newly qualified paramedic and completed a further three years' experience as a Band 6 (or equivalent) paramedic and attained Level 7 capability in paramedic areas of practice. Completed and been signed off formally within the clinical pillar competencies of the Advanced Clinical Practice Framework. Has the ability to assess and triage patients, including same day triage and, as appropriate, provide definitive treatment (including prescribing medications following policy, PGDs, NICE and local clinical guidelines and local care pathways), or make necessary referrals to other members of the primary care team. Has the ability to manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action. Can demonstrate complex decision-making skills to inform the diagnosis, investigation and complete management of episodes of care within a broad scope of practice. Can demonstrate a personalised care approach and population centred care approach to enable shared decision making with the presenting person.

Ability to communicate complex and sensitive information effectively with people at all levels by telephone, messaging in clinical system, email and face to face.
