-Obtain attending/referring MD and CHNE MD signatures on hospice certifications within 2 days of admission. This may require travel to the physicians' office to obtain the signature.
-Document signatures in Suncoast
-Update patients chart with signed hospice certification.
-Coordinate with other teams regarding the certification procedure.
-Ensuring consents are accurately signed and documented.
Education/Experience:
Any combination of education and experience that would provide required skills and knowledge for successful performance would be qualifying.
Typical qualifications would be equivalent to:
-High School Diploma or equivalent
-A minimum of 1-year experience in an office environment
-Experience in a health care environment preferred
