We are looking for a senior Mental Health Practitioner, with leadership experience, to join our PCN as a Band7/8a equivalent (depending upon experience), to act as the professional lead for the team. You will be a qualified Nurse or Allied Health Professional, with the relevantprofessional registration (NMC or HCPC). You will be employed directly by the PCN.

You will be responsible for providing clinical leadershipfor our mental health team who sit across both practices within our PCN.

Part and full-time applications will be considered though weanticipate you would need to be working at least three days a weekto establish effective relationships with the mental health and widerteams. You will have a key role insupporting pre-referral discussions with secondary care and supporting anytransformation work in this field.

Your experience will have been gained within primary care oranother healthcare setting. You would beworking in a diverse and very supportive clinical team with the emphasis ontraining.

This is an evolving role with huge scope for development. It is a fantastic opportunity for thesuccessful candidate to develop their clinical skills in a positive andexcellence-focussed environment.

Main duties of the job

Your primary role will be to provide clinical leadership to the team whilst also managing your own MH clinical workload. You and the team will be supported by a lead GP as well as work collaboratively within the larger multi-disciplinary team which has a reputation for being very mutually supportive and encouraging professional development. Ideally you would be an independant prescriber or be prepared to complete the relevant training.

You will have a vital role in supporting colleagues to provide an excellent primary care first contact for patients experiencing poor mental health in order that they access timely, holistic and evidence-based support based on patient need. You will enjoy building partnerships to find new and pioneering means of strengthening the primary care mental health offer, working with multiple partners across the system. You will work closely with the social prescribing team as part of a PCN MH and wellbeing provision.

About us

The PCN patient population is circa 40k spreading across a large area of mid-west Cornwall from our glorious coastline to our heritage moorland and mines. There are two member practices: Carn to Coast Health Centres and Godolphin Health. Our PCN is innovative and embraces 'Modern General Practice', being one of the early adopters in SW England of the Artificial Intelligence powered triage system KLINIK. A system designed to support clinician triage, patient navigation and improve both the patient and staff experience. Our mental health team support the practice duty teams in managing on-the-day demand and there may be opportunities for triaging patients utilising Klinik, as well as supporting those patients who can be effectively and appropriately managed within primary care with clinically required follow-up.

The post holder will be well supported within the PCN. Our PCN already employs two mental health practitioners so you would be joining an established team. This role is directly employed by the PCN.

Job responsibilities

· To provide clinical leadership to the PCN mentalhealth practitioner team

· To provide a lead role in supporting the PCN intriaging mental health concerns raised by ‘on the day’ patient requests,providing highly specialist advice and support within GP surgeries.

· To review patients presenting with mental healthissues, consider a range of options regarding treatment interventions providingthem with highly specialised advice concerning care when appropriate andliaising with GPs and MH teams.

· To offer brief interventions as needed problem solving, decision making, building resilience, building on personalstrengths, risk de-escalation, medication advice and prescribing if aregistered NMP. (Training will be provided as required).

· Working as a member of the primary Care MDT andtaking responsibility for the development and implementation of specialistmental health training to the PCN.

· To provide a navigation role to help GPs makeappropriate referrals to the relevant community and secondary care services –bridging the gap between primary care services such as IAPT and secondary care,working in partnership across the system.

· To schedule regular team meetings and attend thepractice MDTs as appropriate.
