Job summary
Colne Valley Primary Care Network (PCN) Ltd has an exciting opportunity for a qualified pharmacy technician to join our well-established PCN pharmacy team, to support our 6 GP practices across the Colne Valley with medicines optimisation and the management of long-term conditions. You will be based at The Pump House Surgery, Earls Colne but will be required to travel between all of the GP practices within our PCN. This role is offered on a full-time basis for hours per week over 5 days.
Main duties of the job
Supporting the role of the clinical pharmacist at primary care network level. Providing technical and administrative support to the clinical pharmacists and GP surgery-based clinicians. Working under the supervision of the lead clinical pharmacist, to ensure the safe, accurate and timely supply of prescribed medication to patients. Supporting the PCN and practice teams with queries related to medication. Carrying out medicine switches in GP practices in line with practice and ICB agreed specific protocols reviewing patients records, amending records, and informing patients. Carrying out housekeeping whilst in patient records dose optimisation and medicines synchronisation. Using and maintaining information systems and databases relevant to the position including incident reporting, medicines optimisation, prescribing data, cost savings and patient outcomes. Liaising with other pharmacy stakeholders ICB medicines optimisation team, community pharmacy, and hospital pharmacy teams.

The Colne Valley Primary Care Network is a collaboration of six GP practices serving approximately 54,000 patients, from the Kelvedon, Finchingfield, Halstead, Earls Colne, Coggeshall, and Hedingham areas.

Have strong experience in either a community pharmacy or a primary care setting; Be confident in analysing and presenting information to health professionals; Have experience of implementing and promoting cost-effective strategies; Have the ability to interpret complex clinical information and present it in suitable formats to a variety of people from professional and non-professional backgrounds.-
