We currently have a number of DistrictNurse vacancies across our District Nursing Networks.The established teams of qualified District Nurses, Senior Nurses, CommunityStaff Nurses, Nursing Associates and Healthcare Support Workers workautonomously to provide high quality nursing care to patients predominantly ina place they call home or a clinic setting.

Main duties of the job

Working as part of the District Nursing team you will develop extensive clinical assessment and case management skills. District Nursing teams work closely with their community colleagues to maintain patients independence at home by utilising their excellent networks with our partners and voluntary agencies in local communities, planning and delivering complex packages of care.

The post holder is responsible formanaging and leading a team to ensure provision of high quality care toclients across the district. Thepost holder will work within a District Nursing team utilising theiradvanced clinical expertise, decision making and problem solving skills toprovide skilled nursing care for patients within their own home or clinicsetting.

The post holder will beresponsible for managing a caseload of patients, for the assessment of holisticneeds and the implementation of management plans.

The post holder will have a responsibility to empower team members todevelop within the organisation by demonstrating the principles of leadership.

Act as an autonomous practitioner, assessingpatients physical, social, psychological, emotional and spiritual needs todevelop a holistic care plan. Working in collaboration with other healthand social care professionals and voluntary agencies to provide a high qualityservice. If legally authorised, prescribe medications from the agreedformularies, based on efficacy, safety and cost.
