Job summary

An exciting opportunity has arisen to workas a Breathe Co-ordinator (Smoking Cessation) to support our patientsstruggling to quit smoking. This collaborativenew role works in system partnership between Healthy Lifestyle Solutions CIC, BD4+Primary Care Network, BD4+ Community Partnership and Bradford Council LivingWell service. The post holder will work with patients belonging to BowlingHighfield Medical Practice, Low Moor Medical Practice, Rooley Lane MedicalPractice and Tong Medical Practice.

Main duties of the job

Breathe Co-ordinator will work fromclinical records and identify people who would benefit from quitting smoking.They will then offer very brief interventions over a period of time to supportthem either to access the Living Well Swap to Stop Smoking offer or accessing aSwap to Stop 3-month free vape. Along the way it is envisaged that they willneed other social support which can be provided by the social prescribing teamor by signposting to relevant services.

Thepostholder will utilise different approaches to behaviour change such asmotivational interviewing, coaching and Making Every Contact Count (MECC) tosupport patients to start their quit attempt.

Breathe Co-ordinator alongside other keyservices provided by Healthy Lifestyle Solution CIC and the wider BD4+ Teamwill establish group interventions targeted at specific groups , pregnantpeople who smoke, and older people with COPD.

Breathe Co-ordinator will also support thedevelopment peer-led groups and help people draw on their lived experience to accesscommunity resources to enable each other to improve their lives and makehealth-related behaviour changes.

About us

HealthyLifestyles Solutions (CIC) is a Voluntary Community Service organisation working forthe BD4+ Community. Two key strands of the organisation which include RestoreRecovery College & Social Prescribing.

Restoreis a well-respected and established College set in the heart of a busy andhighly populated location of BD4. It provides a range of recovery, coaching andtraining services for adults within the community, predominantly for those withmental health issues. Restore is closely linked with GP practices and arehosted at Tong Medical Practice alongside their premise at Bizspace. Restorehas a strong partnership working ethos, main partners include BD4Plus PCN andCommunity Partnership, Statutory services, Mental health, VCS sector, BradfordCollege, Local Authority as well as Private businesses. Working in this way, increasesthe opportunities available to the local community and ensures an easy routefor people to get the best holistic support possible.

This exciting new post will sit withinthe Social Prescribing team offering team support to the post holder and theywill work on the same computer systems as the Social Prescribers and thePrimary Care Team. It is a collaborative project between the BD4+ Community Partnership, BD4+ PCN and Bradford Council.

4.The Breathe Co-ordinator will also support thedevelopment peer-led groups and help people draw on their lived experience to accesscommunity resources to enable each other to improve their lives and makehealth-related behaviour changes.

5.People will be reviewed regularly (groupreview) and will have the option to graduate from the service, with thepromise that they can return to the group (through a process mirroringpatient-initiated follow-up) should they feel they need to access support againto keep on track.

6.Provide personalised support to individuals,their families, and carers to take control of their health and wellbeing, liveindependently and improve their health.

7.Develop trusting relationships by giving peopletime and focus on what matters to me.

8.Provide very brief advice at events within thecommunity including schools, places of worships, community centres/hubs andtargeted health events. They will also signpost people where to get support.

9.Take referrals from a wide range of agencies,including PCNs GP practices, MAST, and multi-disciplinary team such aspharmacies, hospital discharge teams, allied health professionals, fireservice, police, job centres, social care services, housing associations, andvoluntary, community and social enterprise (VCSE) organisations (list notexhaustive) and act on these in a timely manner.

10. Workclosely with the Public Health Living Well Service to support their TobaccoControl agenda.

11. Workclosely within the PCN with key clinicians as well as non-clinical teammembers.

12. RepresentBD4+ Community Partnership and the Core20plus5 health intervention which istackling health inequalities out in the community and at associated meetings.

13. Postholder will be proactive in developing strong partnership links with all localagencies to encourage referrals, recognising the need to be confident in theservice to make appropriate referrals.

14. Thepost holder will be working in a developing environment, and he/she willtherefore be expected to undertake other appropriate duties as required.

15.Maintain accurate written records and monitoringand evaluation of the service examples include Swap to Stop and Core 20+5monitoring.

Person Specification

Experience

Essential

Understanding of the wider determinants of health, including social, economic, and environmental factors and their impact on communities individuals, their families, and carers. Understanding of, and commitment to, equality, diversity, and inclusion. Knowledge of IT systems, including ability to use word processing skills, emails, and the internet to create simple plans and reports.

Desirable

Experience of supporting people, their families, and carers in a related role (including unpaid work) Experience of working with the VCSE sector (in a paid or unpaid capacity), including with volunteers and small community groups Experience of partnership/collaborative working and of building relationships across a variety of organisations Local knowledge of VCSE and community services in the locality Knowledge of how the NHS works, including primary care

Qualifications

Essential
