Street Outreach Drug and Alcohol Practitioner - Fixed Term 12 Month Contract (Maternity Cover)

This post is Monday to Friday but will incorporate some early starts and some later finishes on a rota basis (5:30am-12:30pm and 10am-6pm).

About the role...
An exciting opportunity has become available for position of Street Support Recovery Co-ordinator based within our Forward Leeds team.

Street Support Recovery Co-ordination is a diverse and rewarding role, key duties you will undertake are:
  • Manage a caseload, devising, implementing, and reviewing on-going recovery and treatment plans.
  • Facilitate and promote individuals progression from initial engagement into structured treatment.
  • Work in a proactive role to minimise and prevent rough sleeping and begging within Leeds by engaging and maintaining clients into treatment.
  • Reduce substance related harm to individuals and the wider community.
  • Working collaboratively with key partners and colleagues across the street support partnership to achieve the best health and wellbeing outcomes for individuals.
About you
You will have a passion for making a difference and fair chances for all should be at the core of everything you do.
  • You will have an NVQ Level 3 in Health & Social Care (or above or equivalent) or willing to work towards this in post.
  • You will have a fantastic understanding of and ability to implement interventions, including brief interventions MI, PSIs, CBT and ITEP, experience of delivering 1-2-1 and  therapeutic interventions.

Who will you be working with.
You will be working as part of our Forward Leeds Team. The team is a multi-disciplinary recovery team comprising of colleagues from diverse professional and personal backgrounds. The team includes Recovery Coordinators, Medics, Housing teams and Criminal Justice workers , Individual Placement Specialists and Volunteers. We are a supportive and welcoming team, who are passionate about supporting and promoting recovery for those we care for and within our local community.
