Job summary

Following recent investment in the local Tissue Viability Service within Somerset, an exciting opportunity has arisen for a Band 6 Tissue Viability Specialist Nurse, within the community team.

This post is to cover the Bridgwater and North Sedgemoor area.

Somerset NHS Foundation Trust is formed from recent mergers of Musgrove Park Hospital, Yeovil District Hospital and Somerset Partnership (Community Trust); this is therefore a great time to join the specialism, as we look to transform the tissue viability provision across the healthcare system within Somerset.

The successful candidate would ideally have experience of elements of the healthcare environments within Somerset and the digital systems used.

As an independent worker, excellent communication and organisational skills are essential to achieving the high standard of evidence-based care expected by the Trust.

This is a 30 hours per week role; hours and base can be negotiable for the right candidate.

Main duties of the job

This includes:

To provide specialist advice and support to healthcare professional in their care and management of patients with complex wounds and other tissue viability issues.

To support patients and their primary healthcare providers in a variety of settings, including Community Hospitals, District Nursing (in the patient's own home), Mental Health units, GP Practices, clinic settings and Nursing Homes.

To provide advice and guidance on use of pressure relieving equipment and pressure ulcer preventative strategies for complex patients.

To signpost colleagues to other specialist services in line with the needs of the patient and to utilise appropriate clinical pathways.

To support appropriate referrals to secondary care to optimise the patient's health outcomes.

Support the educational provision in relation to the discipline.

Work within Somerset Foundation Trust Values and core standards whilst maintaining a service to colleagues and service users.

Work flexibly across the Trust, as required.

Has proven experience in managing complex wounds,

Has proven experience at a senior staff nurse level - including supporting junior colleagues,

Ability to use specialist wound therapies, such as Doppler assessment (including toe pressures), compression therapy, Negative Pressure Wound Therapy and Larval Therapy,

Act as a role model

Demonstrates personal qualities key to the role - such as compassion, respect, listening,

Ability to prioritise own workload and to keep deadlines with minimal supervision,

Ability to work under pressure,

Advanced decision making skills,

Willingness to use technology to improve standards of care and support to our patients.
