What Will You Work Life Look Like?

In this role, you will provide comprehensive assessment, care planning and treatment coordination for designated clients and their families. Working collaboratively with other health care providers and the multidisciplinary team, you will provide direct psychosocial care, counseling, system navigation and advocacy, education and linkage to relevant specialized complex care resources while facilitating client self-management.

As the Social Worker, you will be responsible for providing and developing social work services to maintain or improve the health and social well-being and functioning of the identified population.

Scheduling Information:

Shift ties are from 08:00 to 16:00 hours. This is a Casual opportunity. Casual means there are no guaranteed hours and work requirements can vary from 0 - 37.5 hours per week. Many careers with us begin with casual employment and lead to permanent or temporary career opportunities.
