An exciting opportunity has risen to work in our bed-based unit at Potters Bar Community Hospital. The ward is a 29 bedded rehabilitation unit working as part of Herts Division Bedded Unit for Central London Community Healthcare.

Main duties of the job

We are looking for a dynamic Band 6 Registered Nurse with experience in acute or community hospital.

The successful applicant will need to be able to demonstrate trust values and have a passion for working with patients during their rehabilitation enabling them to regain maximum self-sufficiency and return home. Applicants must be able to exhibit leadership, motivate and support staff towards continuous learning in the work environment.

You need to be active, innovative, diligent and possess excellent written and verbal communication skills.

Job responsibilities

To be responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care To work in a collaborative manner and liaise with other professionals and agencies to ensure a high quality service for patients To lead and monitor the performance and work of the nursing team To provide a stimulating environment for patients and carers To ensure the effective running of the Ward in the absence of the Ward Manager.

Person Specification

Qualifications

Essential

Registered Nurse with current UK NMC registration Post-registration qualification in older people's nursing or equivalent. Evidence of continuing professional education and training at diploma/degree level Qualification in team / service management or prepared to work towards. Mentor or practice based learning training

Experience

Essential

Evidence of recent post-registration nursing experience in older peoples' nursing. Experience of negotiating and liaising within a multi-disciplinary team. Experience of working with people with complex needs. Experience of coordinating and managing a team. Experience of teaching others and mentoring students. Experience of evidence-based care. Experience of change management

Skills & Knowledge

Essential

Knowledge of NMC Code of Professional Conduct Knowledge of Personal Development Planning. Knowledge of older peoples' nursing, CQC standards, GSF, relevant NSFs and other national standards Knowledge of the audit process Knowledge of evidence based practice Knowledge of clinical governance & clinical supervision. Knowledge of managing difficult situations and conflict resolution. Knowledge of how equal opportunities can be implemented in practice. Has a range of clinical skills and expertise relevant to continuing care of older people. Able to lead and motivate a team and implement change Able to work autonomously, planning and prioritising own workload and that of others Able to delegate work effectively and safely. Effective negotiation skills Able to deal with conflict and manage appropriately. Ability to assess a situation and act appropriately. Ability to communicate effectively both verbally and in writing Able to identify learning and development needs and actively seek ways of meeting those needs. Basic computer skills. Ability to manage and appraise staff Ability to problem-solve and demonstrate clinical reasoning.
