What are Home Teams?

Home Teams is the first part of a wider programme of change to how community services will be delivered in the months and years ahead. This wider programme of Community Transformation is the most significant change to community services in our region in forty years.

Home Teams bring together existing workforce from within Community Health and Social Care in a more targeted and joined up way of working. This approach integrates staff from Community Nursing, Physiotherapy, Occupational Therapy, Patient Flow, Reablement, Social Work Services, Public Health Improvement and Business Support.

There are eight Home Teams across Dumfries and Galloway; The Rhins, The Machars, Stewartry, Upper Nithsdale, Dumfries North and South, Upper Annandale & Eskdale and Lower Annandale & Eskdale.

Home Teams are in the early stage of development and will deliver on the following functions:

· Prevention, Early Intervention, Self Management and Community Support

· Rehabilitation and Reablement

· Rapid Response

· Palliative and End of Life Care

· Facilitated discharge from hospital (Discharge to Assess)

· Community Based Assessments and Reviews

Single Access Point (SAP) will be the route for all new public referrals into Home Teams.

What is the vision for Home Teams?

Home Teams will be fully integrated teams placed at the heart of the community. Home Teams will work closely with people and their families, friends, Carers and local community resources, using person centred approaches that will help them to live healthy and active lives and live at home for longer.

As a member of a Home Team:

· You will be part of a close-knit integrated team who will use your combined skills and knowledge to provide the most appropriate care and/or support to people living in your area.

· Your team will build on existing good practice to ensure each person’s journey through Home Teams is seamless and joined up.

· Your team will work in a flexible way, adapting the way you work to reflect the changing need and demand of people within your area and across the other Home Teams.

· Home Teams will be given training opportunities to up-skill and develop in order to meet the changing needs of people you provide care and support to.

We are looking for a Band 4 OT/PT Assistant practitioner to join our newly formed Home team within the Stewartry. If you don't meet the person spec for Band 4 we are happy to provide a development opportunity for those meeting the Band 3 person spec. This would mean starting at Band 3 and then following completion of agreed training and completed competencies progression to the Band 4 level over an agreed timescale'

 The Assistant Practitioner will provide a high quality, person centred and holistic approach in supporting an individual to achieve their optimal level of function. The Assistant Practitioner will undertake a range of assessments, plan and alter interventions and provide input within hospital and home or homely environments. The Assistant Practitioner will be expected to work and be supervised remotely at times and will receive caseload delegation from an HCPC registered practitioner. The Assistant Practitioner will be required to work closely with health, social care and third sector professionals as well as family members and carers. The Assistant Practitioner must have a UK driving licence and be able to drive due to the community nature of the role.
