We have an exciting opportunity for a rehabilitation Senior Occupational Therapist to join our Inpatient Rehabilitation Service at Trafford Ward, North Cambs Hospital in Wisbech. This position is working with patients on our inpatient wards to support patients out of hospital and offer holistic clinical assessment, rehabilitation, discharge planning and specialist therapy knowledge.

The successful applicant will work between the intermediate care community team, with patients on the home first pathway, and the in-patient rehabilitation wards as part of the in-reach model. Applicants will be offering holistic clinical assessment, treatment, discharge planning and specialist physiotherapy knowledge. Successful applicants will play a vital role in supporting patient flow through the pathway as well as providing clinical skills and knowledge to the MDT. 

This position offers flexibility, variety, and the opportunity to be involved in the transformation of our intermediate care services.

 This is a new operating model of flexibility, merging our community and in-patient therapy staffing to improve patient outcomes with a seamless therapy pathway . Main duties of the job To be professionally and legally accountable for all aspects for your own and delegated work. To work within codes of practice and professional guidelines To undertake holistic assessment (including those with complex presentations and multi pathologies) making use of specialist clinical reasoning skills To use evidence-based practice, to ensure clinical care is effective and appropriate. To promote independence, recovery, and rehabilitation. To avoid hospital admission, reduce inpatient length of stay, and ensure patients receive care in the most appropriate setting, utilising the principles of the Home First model of care. Organise and manage own time, delegating work appropriately Conduct risk assessments as required. Escalating any changes/newly identified risks. To gain patient and carer consent in all interventions, wherever possible To effectively communicate with patients, carers, and colleagues Working for our organisation
