Job summary
An exciting 12 month secondment/fixed term opportunity has arisen for a Personalised Care Lead.
To support operational teams with the implementation of the comprehensive model of personalised care as per the NHS Long Term plan;
To embed the Personalised Care Operating Model across the trust, building on existing activity to develop both universal and targeted approaches to support and empower people to manage their health and well being.
To support clinical work streams and other key stakeholders to identify opportunities for personalised care transformation.
To engender culture change across the organisation, promoting the key components of personalised care supported self-management, social prescribing, personalised care planning, shared decision-making, and patient choice.
To work with partners across the local health system to develop the personalised care offer in the local health economy.
To assess training needs across the Trust to implement the comprehensive model of personalised care.
To foster good working relationships with Primary Care Social Prescribers/Provider organisations to ensure strong inter-organisational working for the benefit of patients.
Main duties of the job
The post holder will empower patients to live well through better conversations, self-management education, care coordination and facilitating peer support.
Influence staff to embed personalised care planning and shared decision making in practice.
Contribute to the development of the personalised care strategy for the trust.
Undertake observations of clinical services and pathways to identify opportunities for transformation and improvement.
Facilitate the implementation of transformation and pathway re-design identified through improvement activity and feedback.
Work with the collaboratives to ensure full implementation of the personalised care agenda.Prepare and facilitate training sessions and other workshops around personalised care.
Work to tight deadlines and facilitate delivery of multiple projects to agreed quality standards/outcomes.
Gather data and analyse benchmarking information for personalised care trends and identify opportunities for transformation.
Regularly access and utilise internal and external information and data sources to interpret and present information.
Provide relevant, succinct, and clear information in a timely manner on the personalised care implementation plan.
Participate in the development of mechanisms to monitor and evaluate transformation initiatives and measure sustained change.
Collaborate with service leads regarding current practice and processes, which can involve positive changes and challenges regarding service change.

Essential
NMC Registered Nurse/ Registered Mental Health Nurse/ AHP having undertaken a program of study at level 6 enabling registration with the NMC/HCPC Master's level qualification or demonstrable experience Management / Leadership qualification

Attendance at the leadership and development and improvement programme ( days)
Knowledge and Skills
Essential
Understanding of the NHS long term plan and of the current issues facing the NHS locally and nationally. Knowledge of the comprehensive model for personalised care. Research skills/audit skills. Evidence of developing and delivering training sessions and presentations. Demonstrable Change Management/Service Improvement experience. Demonstrable Health coaching experience. Computer literate able to use power point, excel and a basic knowledge SharePoint and experience/knowledge of information gathering systems and be able to analyse and report basic care data. Ability to manage difficult conversations and resolve complex issues.
Desirable
Experience of Personal Health Budgets.-
