Job summary

Are you passionate about providing safe continuity of care of those leaving prison and those involved in the criminal justice system?

This is a full-time permanent position working in the Buckinghamshire countywide drug and alcohol integrated service (One Recovery Bucks) and you will have responsibility for ensuring the seamless continuity of care for those leaving prison.

Ensuring the engagement of offenders with substance misuse issues into treatment on release from custody. Completing relevant assessments and referring to One Recovery Bucks to ensure continuity of care. Arranging bridging prescriptions and that medication is continued in the community on release from custody. Working in partnership with key agencies to ensure service users are supported on their individual recovery journeys and those entering treatment remain in treatment when released. Delivering all elements of One Recovery Bucks including managing a case load

Main duties of the job

Main duties of the job will include:

Working within a multi-disciplinary team, in a community & custodial environment delivering a high standard drug and alcohol recovery service in response to the needs of the service users as part of Inclusion and Midlands Partnership NHS Foundation trust. Providing a link between prison and the Community Services and deliver a range of psycho-social interventions appropriate to the individual to motivate engagement and promote remaining in treatment on release, supporting service users to achieve recovery and positive outcomes, improve health & well-being and reduce re-offending. Working in collaboration with clinical prescribing services in custody and in the community to ensure bridging prescriptions are arranged and that the needs of each service user is met when being released from custody Contributing and work towards the key objectives of the One Recovery Buckinghamshire.

To deliver a strengths-based, recovery orientated and outcome-focussed prison link service for all service users being released from prisons to One Recovery Buckinghamshire with a particular emphasis on high standards and good practice. Ensuring smooth transition for prison releases back into Buckinghamshire. To work as part of One Recovery Buckinghamshire and provide throughcare and aftercare support for offenders with substance misuse issues being released from custody. Provide a link into prisons to identify, assess and motivate individuals, to engage with and enter treatment following release from prison. To carry out assessments, make recommendations in line with the service users needs and refer onto appropriate clinical & non clinical services; following agreed pathways and processes. This will include facilitating the transition of service users between prison and community for the provision of prescribing of substitute medications in respect of prisoners released on temporary licence (ROTL) To work with a range of partnership agencies and prison based services to co-ordinate planning and support for release: in particular to develop close links with the Community Drug and alcohol teams, IOM, Probation / CRC, Police and other partnership agencies. To provide access to services post release from treatment providers in an integrated and collaborative process of Recovery care planning. Person Specification

Qualifications

Essential

Foundation degree / HNC in related fields ( counselling, HR, social work, nursing) or relevant experience in comparable setting

Desirable

Relevant training / Qualification in associated field oCBT oMotivational Interviewing oBrief solution focussed therapy oMapping techniques (ITEP/BTEI) oAuricular acupuncture trained

Experience

Essential

Experience of integrated working practices Extensive experience of working with substance user's
