Job summary

You willprovide exercise based support and advice to patients providing them with a structuredevidence based home exercise plan and be based between Aldeburgh and Woodbridge. All patients will have been through aclinical triage process before being referred to your care. You will be treatingpatients who have been diagnosed with either single or multiple health conditionssuch as musculoskeletal injuries or pathology, type two diabetes, long COVIDsymptoms or patients with persistent pain. Patients will be cleared from anyunderlying health conditions that adversely impact their ability to completeexercise based treatment effectively and safely by a Physiotherapist. You willbe provided with a detailed referral for each patient including their diagnosis,goals and outcome expectations. Sessions will be predominantly class based orif appropriate as one to one sessions.

Main duties of the job

To work in partnership with AHP Suffolk physiotherapists to enable and empower individuals, families and communities to engage in healthy lifestyles as part of service development. To monitor the condition of the patients during classes and one to one sessions and ensure they exercise in a safe and appropriate way. To ensure the workspace you are assigned to is safe and secure for staff and patients, including any digital systems. To participate with the delivery of planned programmes of care to participants with NHS based treatment. To implement treatment plans and progress them with the support of the referring Clinician and other members of the Exercise Therapy team. To monitor the progress of patients receiving exercise based treatment under your care and communicate relevant changes back to the referring Clinician or senior staff member as appropriate. To prepare the treatment area prior to a class or one to one session including visual safety checks, tidying and wiping down, ensuring appropriate equipment is in place and ensuring a comfortable and safe environment. To assist in the day to day administration of the clinical pathways including diary management, preparing documents for sessions, photocopying, filing, data management including maintenance of patients details and use of bespoke IT Systems.

Job responsibilities

Provide exercise based support and treatment to patients who have been diagnosed with either single or multiple health condition such as musculoskeletal injuries or pathology, type two diabetes, Long COVID symptoms and patients with persistent pain. To provide a home exercise plan to all patients that you treat helping towards their long term self management. Set up/clear down and lead classes and one to one sessions within designated AHP Suffolk or partner work sites. To assist in the maintenance of information systems which ensures that the work of all pathways, classes and one to one sessions are recorded correctly. Support all AHP Suffolk staff in providing an efficient and effective Physiotherapy Service within the framework of clinical governance. Person Specification

Experience

Essential

-Working within a relevant job role. -Working with members of the public from a range of ages 16 and above. -Experience of working from a diary/appointment based system. -Basic computer skills and a willingness to use computer based systems. -Experience in coaching the safe and effective use of a number of common items of gym equipment without the need for additional training.
