Job summary

Are you a nurse looking for a new challenge? Do you want to enable patients to enjoy maximum independence and optimum quality of life?

The successful postholder will be accountable to the Senior Virtual Ward nurse and will have continuing responsibility for the nursing care given to patients with sub-acute conditions in identified community settings, without direct supervision. You will be personally accountable for the nursing standards. You will also be responsible for demonstrating procedures for qualified and/or unqualified staff; and will provide evidence-based care, advice and information to patients and carers.

You will be employed by North Middlesex University Hospital (Enfield Community) with primary responsibility at NMUH Virtual Ward. Occasionally the post holder may be expected to work across the wider VW team.

You are are required to hold a full UK driving license and have access to car for work.

Main duties of the job

The successful post holder will proactively manage a caseload of patients requiring complex case management in order to prevent hospital admissions and facilitate early discharge to improve patient health outcomes. Patients requiring case management in the Virtual Ward service will be identified by case-finding in the Same Day Emergency Care (SDEC) unit, Accident and Emergency department, NMUH wards. You may be required to support Rapid Response with Community referrals with come via GPs, London Ambulance Service, 111, Care Homes and Community Nurses.

Job responsibilities

Clinical/ Professional Role:

Provide specialist, high quality, expert nursing care and support for patients who are suitable for Virtual Ward (VW) care in liaison with the multidisciplinary teams (MDT) through assessment and care planning. Initiate agreed range of diagnostic tests Refer to other disciplines/professions when required Support patients to self-manage, monitor their condition, and make informed choices Act as a named nurse for designated patients within the service Ensure that nursing documentation is maintained to required standards and to be a competent user of and participate in the ongoing development of the Trusts planned Electronic Patient Record Promote the development of evidenced based practice. Participate in clinical supervision to agreed standard Ensure high standards of care are given to all patientsBe responsible for gaining intravenous access and administration of prescribed medication for patients requiring short-term antibiotics and for monitoring their care.

Collaborate with medical colleagues in ensuring that appropriate investigations and procedures are performed to complete the patients treatments and that patient, and their families are fully informed and understand the purpose for these.

Advise patients on the promotion of health and the prevention of illness. Person Specification

Registered Nurse with current NMC registration Bachelor's Degree Advanced Physical Assessment Course

Desirable

Independent Nurse Prescriber (V300)

Ability to assess and implement appropriate care plan and adapt plan depending on patient's response to treatment. Ability to assess and identify the deteriorating patient Excellent clinical reasoning skills & decision- making skills Able to work confidently in an independent and autonomous role whilst being aware of own limitations Excellent drug/medication knowledge and able to link pharmacology to patient's treatment and presentation Broad range of knowledge relating to multiple specialties

Experience of working within the Community Nursing Extensive post registration experience in Acute care Experience in ITU, A&E, Acute assessment units would be advantageous Experience of managing a team Experience of managing staff recruitment and retention. Understanding of the role of Primary care in admission prevention and supporting a timely discharge.
