Job summary

We are looking for an experienced Mental Health Practitioner to join our Thurrock PCN for 12 months fixed term.

As part of the NHS long term plan, local areas were asked to realign community mental health services with primary care networks, creating new and integrated models of primary and community mental health care.

PCN's aim to:

Have a key co-ordinating role at the heart of the GP practices and are a fully integrated member of the multi-disciplinary team with regards to response and management pathways Provide a combined consultation, advice, triage and liaison function in accordance with agreed protocols Support children and young people with mild to moderate mental health problems through evidence-based interventions.

PCN's (Primary Care Networks) are a new service designed to support children and young people. The Mental Health Senior Practitioner will be responsible for the assessment and caseload management of clinicians within their GP PCN. They will triage cases, complete a full assessment and manage their workstream. There will manage a caseload of CYP with mild to moderate MH needs. The post holder will work closely with operation manager and Clinical & Strategic Lead, to ensure effective delivery of the new PCN initiative in Essex.

Main duties of the job

This CYP MH Practitioner post is a new post to support the development and delivery of the PCN service in Essex. The post holder will be responsible for supporting the delivery of evidence-based low intensity interventions in GP services, supporting liaison between professionals. Working in partnership with GP's to support children, young people experiencing mild to moderate mental health difficulties and their families in the self-management of presenting difficulties.

The post holder will have experience working with CYP within this framework to support mild to moderate mental health needs. This will include the delivery of individual interventions and group support. The post holder will have experience in managing a clinical caseload and delivering therapeutic and low intensity interventions, as well as experience of case load management to meet service targets and manage waiting list demands.

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High-Cost Area Supplement

This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (minimum payment of £1,192 and a maximum payment of £2,011 per annum pro rata).

This PCN CYP MH Practitioner posts are a new service to support the clinical development and delivery of the Primary Care Networks (PCNs) in Essex.

Post holders would be community-based spending time in primary care settings and other sites that supports access

We are looking to recruit an applicant with expertise in the delivery of therapeutic interventionsto children and young people (CYP) and their families to manage anxiety, low mood, and behavioural problems.

PCN CYP MH Practitioners would be expected to hold a case load of young people with mild to moderate levels of complexity. The post holder will be supporting the development of the PCN's service in conjunction with the service manager and clinical lead. Working to facilitate wider service transformation within the local child mental health system.

The post holder would be expected to work closely with the Service Manager and Clinical lead. They would also be expected to participate in the training opportunities available to them.

The following are the core clinical responsibilities of the PCN mental health practitioner. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels: To work as part of a multi-disciplinary team in a patient facing role to assess and manage patients using expert knowledge To assist in the assessment of new patients with subsequent production and completion of individual care plans To provide specialist clinical support and advice to young people with mental health difficulties To work with patients to support engagement with appropriate services and treatment which may include onward referral to secondary mental health services or signposting to other mental health services, voluntary services and support groups To educate and involve family members and others in treatment as necessary To be responsible for the day-to-day planning of personal workloads To follow departmental policies procedures and guidelines To liaise with the practices and, where practicable, to standardise referral processes across the PCN To triage and also receive referrals and directed patients from within the PCN To communicate effectively with patients, recognising the need for varying methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating

Base

Post Holder will work in Thurrock PCN's . Some travel around the county is also expected. The post holder will be working primarily from their GP Practises within the PCN in the allocated following areas:
