LDA Hospital at Home is an innovative new service being developed under the Thames Valley Provider Collaborative. The Hospital at Home team will focus on young people with moderate to severe learning disabilities AND/OR a severe degree of functional impairment associated with an autism diagnosis who are at risk of home or placement breakdown and who could meet the threshold for inpatient care.

In some cases, the team will help prevent unnecessary admissions and in others, young people will be stepped down from tier 4 care through the LDA Hospital at Home team, shortening their inpatient admission. LDA Hospital at Home maximizes the use of new technology, provides care close to home, reduces admission, length of stay, waiting times and provides earlier discharge from inpatient managing the care pathway, we will be able to ensure that children and young people are cared for in an inpatient setting or via an 'out of hospital' service as close to home as possible. This will:

Reduce the burden on families to travel long distances Enhance continued engagement with community clinical teams Encourage early therapeutic home leave Minimise Length of Stay Avoid admission

Main duties of the job

As the LDA, Nursing Associate in the team you will have a breadth of knowledge across the fields of nursing, providing holistic and person-centred care and support for young people up to 18 years old and in a variety of settings. The LDA Nursing Associate works independently under the leadership of Registered Nurses, working within the sphere of nursing and care and within all aspects of the nursing process.

The role will assist the registered nursing staff (Band 5 and above) and other health professionals to deliver a wide range of nursing interventions including the care and treatment of multiple mental health illness' , young people that may have a learning disability and/ or autism and social vulnerability. It requires strong all-round capability, insights and adaptability in physical and mental health, as well as organisational, communication and inter-personal skills.

Your base will be in the new offices in the Meadow Unit on the Warneford site, there also will be planned visits within the community across the Thames Valley collaborative area. you will be required to work flexibly, whilst planned between the hours of 8am-8pm.

Job responsibilities

- Work on a shared caseload for the team and to be supervised by a registered Nurse- Record own work on electronic patient records Rio- Contribute to Nursing assessments Initial Assessments, Nursing Assessments, etc. (including writing and sharing nursing formulations and nursing recommendations with relevant people- Contribute to the design and implementation of clinical interventions including care planning.- Monitoring and feeding back on the physical and mental well-being of service users, including escalation of care needs when necessary.- Signposting to and working in collaboration with appropriate health services, inpatient and community-based support. Liaising with other professionals outside the immediate MDT including GPs, hospital departments doctors and nurses, social care, education etc.- Deliver, under supervision, specific therapeutic interventions to meet the physical, emotional and psychological, social, cultural and spiritual needs of individuals and carers.- Arrange patient & carer monitoring visits and attend, as part of a planned package of care- Discuss and explain a plan of care with patients and families or carers including the use of Easy Read information.- Write and review care plans under supervision from a registered Nurse and update electronic care records. ( Rio)- Create and update Patient held Care records Health action plans, hospital passports etc.- Undertake Health Education work impact of mental health illness on overall wellbeing and support for individuals and families on recognising signs, symptoms and ongoing triggers.- Monitoring patient medication concordance. Educating patients re possible side effects and monitoring same. Administering of Medication in accordance with regulatory guidelines for Nursing Associates if required.- Sourcing or designing and producing Easy Read Documents- Record Keeping updating initial assessments, updating risk summaries, writing and sending closure letters to service users and their families and GPs- Supervision & Preceptorship including statutory & mandatory training, learning & development, and CPD.- Multi-Disciplinary Team working with the entire LDA hospital @home Team to deliver holistic, evidence based, assessments and interventions.

Registered Nursing Associate on the NMC register. Completion of Nursing Associate foundation degree qualification.
