Job overview

We have an exciting secondment/fixed term opportunity for a registered healthcare practitioner (NMC or HCPC) to join our Community Nursing Service as a Dementia Specialist.

As a service we provide interventions for housebound patients aged 18 and over delivering personalised nursing care to those with complex and long-term conditions. The aim of our service is to enable patients to stay safe and well in their chosen environment and to reduce unnecessary hospital admissions. 

Our patients want to be at home and we want to keep them there.

Main duties of the job

In this role we are looking for someone to support the provision of highest quality dementia clinical care to patients and their carers through personal actions and continuous improvement.

You will work with the community team, identifying those patients under their care who might have dementia and enable them to develop the next steps in their assessment and or care.

You'll also provide leadership, advice and support in fostering a learning culture which values high quality patient care for those living with dementia.

Qualifications

Essential criteria

Registered Practitioner (NMC/HCPC), Evidence of continuing professional development, First Degree (health related subject) Leadership/managerial qualification or equivalent experience, Mental Health qualification or equivalent experience,

Desirable criteria

Additional professional/management qualifications, Non-medical prescriber

Experience

Essential criteria

At least five years’ experience of working with patients with dementia, Leadership and Managerial experience in a variety of community, residential and in-patient settings caring for people with dementia, Supervising and appraising subordinate staff Evidence of participating in organisational change that has led to changes in quality, Evidence of leading projects that promote enhanced care,

Desirable criteria

Evidence of working with dementia patients with complex needs and behaviours, Working with older adults with functional mental health needs and illnesses, Working within a multi-agency setting

Knowledge

Essential criteria

Knowledge of the common features of dementia and its more unusual presentations, Knowledge of the main dementia assessment tools and their use, Knowledge of referral and care pathways for assessment, Knowledge of the role and responsibility of other care providers to patient with dementia, Knowledge of appropriate care interventions relevant to a Community Trust setting Knowledge of relevant medical interventions for a patient with dementia Knowledge of behavioural techniques to manage challenging behaviours in the least restrictive manner, Good working knowledge of contemporary health care issues and professional standards, Understanding of the complex dynamics of a multi-professional environment
